Observed food debris build-up on the floors, sink drain, refrigerator handles and walls in the facility. Floors, handles and walls shall be kept clean. Clean floors and wipe surfaces.

1. Observed ready to eat foods, cheese and condiments stored on top of the kegs in the walk in refrigerator. Food and produce shall be stored on appropriate shelving. 2. Observed lettuce and other produce items stored in a box on the walk in refrigerator floor. Food shall be stored at least 6" above the ground and in appropriate food/produce shelving. Correct immediately.

Sanitizer bucket had a concentration of 0ppm. Sanitizer bucket shall maintain a concentration of a minimum of 200 pmm quat solution. Corrected on site.

Observed broken spots on walls and ceiling of walk-in cooler. All equipment shall be in good condition and well maintained. Repair/replace walk-in cooler. Correct by 8/2025.

Observed grime build-up on deli slicer, per manager the unit is only taken apart once at night and cleaned. Meat slicers shall be dismantled, cleaned and sanitized every four hours or sooner, if needed. MITIGATED ON SITE. Observed mold build-up on the ice dispenser of the soda fountain. All equipment shall be kept clean and free of contamination. Ice dispenser was cleaned and sanitized at the time of inspection. CORRECTED ON SITE.

MAJOR VIOLATION: Observed cream cheese with an internal temperature of 70F. All potentially hazardous food shall be at 41F and below. Cream cheese was voluntarily discarded at time of inspection. MITIGATED ON SITE. Observed items in the 3-door prep unit with an internal temperature of 45-48F. All cold holding shall be at 41F and below. Items were moved to the walk-in to return to proper cold holding temperatures. MITIGATED ON SITE.

REPEAT VIOLATION: Observed CO2 tanks not properly restrained to the wall. All CO2 tanks should be chained/strapped in a way that will prevent them from falling over. MITIGATED ON SITE. Observed the ambient temperature of the 3-door cold holding unit at 50F. All units used for cold holding shall provide an ambient temperature of 41F and below. Have unit serviced, repaired, or turned down to provide proper cold holding temperatures. All items in the top of the unit can be placed on ice baths to maintain proper cold holding temperatures. Correct immediately.

MAJOR VIOLATION: Observed the following items held at 48-53F in the three door cold holding unit. -Chicken -Roast beef -Ham -Turkey -Raw bacon All potentially hazardous food shall be at 41F and below. All items were stored in the unit for greater than 4 hours. Items were voluntarily discard on-site. CORRECTED ON SITE. See violation #35.

MAJOR VIOLATION: REPEAT VIOLATION (7): Observed no Food Safety Manager Certificate available at the time of the inspection. All food facilities shall have at minimum 1 certified food safety manger. Provide a Food Safety Manager Certificate by 2/11/2023. A copy can be sent to [email protected]. Failure to comply will result in a yellow placard. Observed no current food handler cards on-site. Food handlers must have food handler cards and the facility should keep record of all employee food handler cards. Correct by 2/11/2023. Failure to comply will result in a yellow placard.

Observed the three door cold holding unit with an internal temperature of 57F. All cold holding units should have an ambient temperature of 41F or below. Adjust the temperature or have the unit serviced to supply the proper cold holding temperatures of 41F. CORRECTED ON SITE. REPEAT VIOLATION: Observed broken gaskets on the doors of the three door cold holding unit. All equipment shall be kept in good repair. Replace broken gaskets. Correct by 7/2023.
