Observed food in cardboard boxes on floor of walkin cooler. All food shall be kept off the floor at least 6 inches. Employee moved food onto crates so food is stored off floor. Corrected on site.

REPEAT VIOLATION
The Soda machine dispenser holster on the far end of the restaurant bar does not have liquid waste drain line. INstall chase line to prevent liquid waste from dipping on ice. Plug holster to prevent contamination.

Observed reach in cooler with wine bottles and glass water with pooling water on bottom. Inside fan of fridge has accumulation of biofilm. Repair unit to not be leaking and empty/ sanitize.

Observed overfilled container of brussel sprout in the refrigeration drawer. Overfilled containers scrape the top of the unit and cause contamination to food. Do not overfill containers.

Food Safety Manager Certification not available at the time of inspection. Facility must have at least one employee with a valid Food Safety Manager Certificate. Certificate must be maintained on site at the facility and made available upon request.

Observed food debris accumulation on the can opener. Clean and sanitize.

Observed grime accumulation in the soda gun nozzle and holder. Thoroughly clean and sanitize.

Observed fish defrosting in vacuum packaged bags under cool running water in the sink. Per packaging, product
must be kept frozen or removed from the packaging and thawed in the refrigerator.

Observed incomplete consumer disclosure and advisory for raw/undercooked food. Facility must provide a written consumer disclosure and advisory statement on the menu. Menu items that may be cooked to order or contain raw or undercooked ingredients must be marked with an asterisk that corresponds with the statement below.

Observed no liquid waste chase line connected to the soda gun holders. Install chase line to prevent liquid waste from dipping on ice. (Plug hold to prevent contamination immediately)

Observed dust and debris on the window sill in the kitchen. Clean.

Observed significant deterioration to the floor in the lower walkin cooler. Water pooling, large cracks with water seeping through. Plan to replace floor in walkin in the near future.
