Observations:
1. No ants were observed in the facility. The food debris on the floor and microwave area was cleaned up. In compliance.
2. Mop sink was clear and accessible. Per Manager, employees have been trained to discontinue dumping mop water outside and to use the mop sink. In compliance.
3. Refrigerator/freezer combo was not being used to store food and is in the process of being repaired. Expected repair date is tomorrow, 6/9/2023. Call inspector when ready for reinspection of this unit.

Per employee, the password was lost. Inspector assisted with reseting password for employee to access course. Certification MUST be on site during next routine inspection (approximately 6 months from today). Failure to comply will result in a re-inspection fee of $417.

MAJOR VIOLATION - Observed approximately 30+ of ants in dry storage area on floor, and on the table with microwave. Food debris was observed on floor, table, microwave, and exterior of refrigerator, attracting the ants. All food facilities must be kept free of vermin. Per employee, pest control service sprayed outside the building yesterday (6/6/2023) for ants. Re-inspection required. Inspector will return on 6/8/2023 to ensure food facility is free of ants and surfaces are free of food debris. Do not use microwave until reinspection has been conducted. Failure to comply will result in reinspection fee of $417. Note: Ants noted in dry storage area only, not in customer self service area. No food contamination observed. Another microwave in the customer self service area is available for use. MITIGATED ON SITE.

REPEAT VIOLATION - Observed mop sink cluttered with unused equipment. Per employee, mop water is disposed of outside. Discontinue this practice immediately. Wastewater must be disposed of properly into a sanitary sewer. Clear mop sink ASAP to ensure it is accessible for the disposal of wastewater. Comply by 6/8/2023.

MAJOR VIOLATION - Observed employee washing dishes in 3-compartment sink and skipping the sanitization step. When washing dishes and utensils in the 3-compartment sink, the wash, rinse, sanitize method must be utilized. Inspector educated employees on proper warewashing method, and all dishes/utensils in drying rack were sanitized in 200ppm quaternary ammonium solution for 1 minute. CORRECTED ON SITE.

Observed cardboard boxes with single use cups stored on the floor in the dry storage area. All food and utensils must be stored 6" off of the floor to protect from contamination.

MAJOR VIOLATION - Observed reach in refrigerator/freezer combo with an ambient temperature of 49*F, and wet rags on the bottom shelf, indicating an issue. Whipped butter, cream cheese, and yogurt probed at 50-53*F. Potentially hazardous food (PHF) must be maintained at or below 41*F. All PHF inside refrigerator was voluntarily discarded at time of inspection. MITIGATED ON SITE. Do not use refrigerator until refrigerator is repaired and re-inspection has been conducted. Comply by 6/21/2023. Contact inspector if ready for reinspection sooner. Note: Freezer side of the unit is working properly, and other refrigeration is available until this unit is repaired.

On-site to verify enrollment in Food Safety Manager course and purchase of test strips.
1. Proof of enrollment provided to inspector. Ensure certificate of completion is available during next routine inspection.
2. Quaternary ammonia test strips on site. Employees instructed on how to use test strips. Ensure concentration for sanitizer solution and warewashing is 200ppm quaternary ammonia. Per employees, chlorine sanitizer (bleach) will not be used in the kitchen. Chlorine test strips not required.

Provide test strips for both chlorine and quat ammonia sanitizers to verify sanitizer is prepared at the proper concentration (chlorine at 100ppm and quaternary ammonia at 200ppm).

Observed a pile of soiled cloths on floor in warewashing room. Per employee, cloths are too soiled to be laundered and will be thrown away. Dispose of cloths or provide receptacle for soiled cloths.

Observed unaccessible mop sink cluttered with unused equipment. Per employee, mop water is disposed of outside. Discontinue this practice immediately. Wastewater must be disposed of properly into a sanitary sewer. Clear mop sink ASAP to ensure it is accessible for the disposal of wastewater.
