Maintain walk-in refrigerator at 41*F or below. Should walk-in refrigerator discontinue working again, facility must immediately close until repairs are made and unit is holding at 41*F. Facility has no alternate refrigeration units and may not be operating without a working walk-in refrigerator.

MAJOR VIOLATION - Observed ambient temperature of walk-in refrigerator at 63*F. Raw chicken probed at 61-63*F. Per manager, unit went out yesterday and chicken was discarded. Chicken currently in unit was brought over from freezer to thaw. Discard all product in unit and thoroughly clean and sanitize interior. Have walk-in refrigerator repaired to hold at 41*F or below. MAJOR VIOLATION - Observed cole slaw on the prep line probed at 55*F. Per employee, cole slaw had been in the walk-in. Cole slaw was voluntarily discarded during inspection. FACILITY IS CLOSED due to lack of refrigeration.

Observed no water available at the hand wash sink near the drive-thru window. Hand wash sinks must be supplied with water between 100-108*F at all times. Additional hand sinks in facility are not conveniently located for employees working the drive-thru window to wash hands.
Observed hand wash sink next to the ice machine to be leaking water from both the faucet and the foot pedals on the floor. There is standing water around the hand wash sink and ice machine due to the water leak.
Observed ice build-up on the floor of walk-in freezer. Liquid waste drips from the condenser onto the shelving and the floor. This is a slip and fall hazard and a potential source of food contamination. (Note: this has been on ongoing issue since 2021) Note: employees have placed trays under the condenser unit to collect liquids that may drip down.
Observed the door gasket around the walk-in freezer door to be cracked, damaged and allowing air leakage. Door does not fully close.

Per manager, the facility is serviced several times a week by a licensed pest control operator, and both the exterior and interior of the facility have been chemically treated. Note: No live roaches were observed in the facility at the time of inspection. Continue with regular pest control service and follow all recommendations from pest control company.

Comments:
Observed broken and missing floor tiles and cove base tiles replaced with new tiles in multiple ares of the facility including the main cook line, prep areas, ware wash area and dry storage. Observed no water seeping from beneath tiles in the kitchen. Observed light fixtures above the cook line and above the chicken make table to be repaired and providing adequate lighting for food preparation and handling. Observed new wall tiles installed next to the shelving in the warewash room and repairs to the tiles above the mop sink. Per employees, roof leaks have been patched and no water seepage occurred during the last rainstorm. (Conditions were dry on the day of re-inspection.)

Comments:
1) REPEAT VIOLATION (4th) - Observed broken and missing floor tiles and cove base tiles in several locations throughout the facility, including the main cook line, prep areas, ware wash area, dry storage and the walk-in. Observed standing water pooled in corners and under equipment. Observed water seeping up from beneath tiles in the kitchen.
Per CAL Code 114268. Floors (a)... floors shall be SMOOTH and of durable construction and nonabsorbent material that is EASILY CLEANABLE. 113767. Easily cleanable "EASILY CLEANABLE" means a characteristic of a surface that allows effective removal of soil, FOOD residue, or other organic or inorganic materials by normal cleaning methods.
Food facilities shall be kept in good repair.
Floor must be repaired/replaced. (Note: flooring must be replaced like for like or plans must be submitted to Placer County Environmental Health for approval. Floor must be properly sloped to drain, floor sink must be installed flush with the floor surface (no rim)

Per CAL Code Article 1. Protection from Contamination 113980. Requirements for food. All Food shall be protected from...overhead leakage, or other environmental sources of contamination. 113984. Food preparation area, protection from contamination. (e) Overhead protection shall be provided above all FOOD PREPARATION, FOOD display, WAREWASHING, and FOOD STORAGE areas.
Facilities shall be kept in good repair and food protected from contamination. Repair all roof leaks, repair ceiling/replace damaged portions.
Note: A re-inspection of these repeat violations occurred on Feb. 7, 2022 at which time facility management stated that the owner had scheduled repairs to be made. To date no repairs or corrections have been made and the severity of the violations has increased.
3) REPEAT VIOLATION - Observed missing tiles/wall sections above the mop sink and next to the shelving.
Per CAL Code 114271. Walls and ceilings (a) Except as provided in subdivision (b), the walls and ceilings of all rooms shall be of a durable, SMOOTH, nonabsorbent, and EASILY CLEANABLE surface.
Facilities must be maintained in good repair. Replace missing wall tiles.
4) REPEAT VIOLATION (4th) - Observed water seeping out from under tiles and stagnating between tiles in the kitchen. Due to missing grout between tiles throughout facility, water is seeping and collecting under tiles. Floor is no longer smooth nor easily cleanable. Replace grout in floor to create a smooth, washable surface that does not trap water, food, debris, etc.
5) Observed area between the front counter and service area to be dry. Per manager, there had been a clog in the line of the warming oven which caused water to overflow onto the floor. CORRECTED
6) REPEAT VIOLATION - Observed water damage to the light fixture above the cook line and above the chicken make table. Light fixtures are not working and there is not adequate lighting in these areas. It is also a potential electrical hazard.
Per CAL CODE Article 2. Lighting 114252. Lighting (c) Except in server stations where FOOD is prepared, at least 50 foot candles at a SURFACE where a FOOD EMPLOYEE is working with FOOD or working with UTENSILS and EQUIPMENT such as knives, slicers, grinders, or saws where EMPLOYEE safety is a factor. Repair light fixtures.
6) REPEAT VIOLATION - Observed water pooling in the light fixture in the dining room. This is a potential safety hazard.

Items probed: Macaroni and cheese - 54*F, 3 trays of chicken tenders - 55*F, shredded cheese 53*F, 2 boxes of raw chicken pieces 49-52*F. All PHF must be held at 41*F or below. Manager voluntarily discarded all food being held in the walk-in. A service call has been made to have the unit repaired. In the meantime do not use the walk-in refrigerator until holding at 41*F or below. Facility must use the walk-in freezer or place items in an ice bath. A re-inspection is required. Observed cole slaw stored on top of ice in an ice bath in the walk-in, holding temp at 45°F. Please make sure to submerge tubs of food held in ice up to the level of the food to ensure cold temperatures throughout. CORRECTED during inspection.
