Observed hole in ceiling near walk-in refrigerator door. Repair.

Observed multiple drainage pipes below the rim of the floor sink located under the soda syrup area. Equipment that discharges liquid waste shall discharge through an air gap into a floor sink. Shorten/adjust drainage pipes so there is at least a one inch air gap between the waste pipe and the rim of the floor sink.

Observed bags of ice packaged at facility not properly labeled. Sticker or stamp bags with contact information of facility (Name, address).

Observed duct tape on damaged ice scoop. Utensils must be in good repair. Ice scoop voluntarily discarded at time of inspection. CORRECTED ON SITE.
