Observed walk-in refrigeration door was unable to close properly by itself. Ambient temperature was observed to be at 38*F. Repair door to allow for refrigeration unit to close properly. Repair man was on site, identified a door hinge needed to be replaced, showed new replacement part to inspector.

No Food Handlers Cards available for staff members at this time. Have all staff members that prepare food items obtain handler cards by next inspection. Food Managers certification is current and available.

Observed no lids/covers available for dry goods storage bins. Keep all food items covered when not in use to prevent possible contamination.

Ware washing machine was observed to be at 0 ppm at the start of the inspection. Detergent container was also observed to be empty. Unit line was submerged in chlorine container and detergent replaced. Observed unit was able to reach 50 ppm CORRECTED ON SITE.

Observed several flies in the kitchen. Take the following precautions to help eliminate flies: 1) Clean and sanitize damp, dark areas in the kitchen (floor sinks, underneath the dishwasher/3-comp sink). 2) Keep all doors closed as much as possible during the day. (Weather strip back door). 3) Install approved light traps with sticky boards, taking care not to risk contamination of food and food contact areas. 4) Contact a licensed pest control service and begin regular service if the above steps do not take care of the issue.

Observed warewash machine at 0ppm chlorine. Have unit serviced to produce 50ppm chlorine during rinse cycle. In the meantime, sanitize all dishes and utensils in the 3-compartment sink.

Observed carne asada to be at 81*F on deli cold holding line at 1 pm. Informed that the carne was cooked at 10 am. All foods shall cool down with-in four hour window to prevent possible bacteria growth. Move carne to cold holding to bring down internal temperature.

THis is a re inspection to the routine inspection conducted on the same day 7/14/2022. Routine inspection resulted in closure due presence of live cockroaches.
The following was observed:
1) 4-5 dead cockroaches were observed on the floor surfaces in the walk in unit.
2) One dead cockroach was observed on the floor surfaces in front of the cooking equipment.
Dead cockroaches were swept up and floor surfaces were cleaned and sanitized during re inspection.
Facility provided pest control receipt dated 7/14/2022. Continue to provide pest control as needed.
Facility was approved to re open.

Container of salsa and container of cheese were stored directly on top of open containers of lettuce and cheese inside the prep unit on the cookline. Open containers of food shall not be stacked on top of each other. Replace large containers with smaller containers to allow all food items to be stored properly inside prep unit.

Observed in use utensils stored in 80 degrees F water. In use utensils shall be stored in 135 degrees F water. Correct immediately.

Observed grime build up on the sides of the deep fryer in the back area. Clean and sanitize all cooking equipment.

Observed back entrance door to be left open during first part of inspection. Keep doors closed to prevent vermin entry.

Observed 2 live cockroaches in the kitchen area underneath the counter where clean plates are stored. Also observed 1 dead cockroach inside the walk in unit. Eliminate all vermin from the facility. Manager was unable to provide a pest control receipt for service. Provide pest control services for the facility. Receipt shall be made available. Facility is closed until all vermin have been eliminated from facility.
