Observed raw salmon wrapped in plastic stored above wine bottles in walk-in cooler, please store raw meat items on lower racks below any ready to eat items. Corrected on site. Observed stalks of asparagus rubbing against the interior side of the lid in top cold holding unit. Please store items in a manner to avoid rubbing against lid. Items were placed horizontally in a plastic container.

Observed overfilled containers of tofu and sliced melon in top cold holding unit near cook line, temperature of melon was 44*F and Tofu at 45*F for items located on the top. Please fill containers up to fill line to maintain food items at or below 41*F. Items were removed from top of containers and filled properly, corrected on site. Observed raw chicken in plastic containers inside cold holding drawers under grill. Ambient temperature on unit's digital thermometer read 45*F, plastic containers were in ice and chicken has a temperature of 44*F. Items were moved to walk-in, please make sure temperature of unit reads correctly and place a thermometer inside of drawers for more accurate reading.

Observed tape being used on middle prep surface and fooil covering some of the prep surfaces near cook line. Surfaces must be smooth and easy to celan. Please use a reusable material to cover prep surfaces. Observed mold in ice machine, please clean ice machine regularly. Corrected on site. Observed food debris on racks in walk-in cooler and in dry storage area. Please clean racks and any spills in a timely manner to avoid any build-up.

Over all, facility needs to improve its cleaning practices and make sure employees are properly trained. Food handler cards are available and food safety manager certificate is current and posted.
Please address the following to avoid any REPEAT VIOLATIONS and possible re-inspection fees and YELLOW PLACARD:
-Clean door handles and gaskets of all refrigerators and freezers
-Organize walk-in and clean racks
-Make sure sanitizer is available at all times at proper concentration, use test strips
-Make sure soap and paper towels are available at all times at all hand sinks
-Clean prep surfaces and remove any torn or ripped aluminum foil
-Clean floor and walls in hard to reach areas.

Observed wooden prep table covered in plastic, plastic is deteriorating at edges. Please make sure table is not used as cutting board and replace plastic once it starts to deteriorate. Consider replacing table with smooth and easy to clean surface that is non-absorbent.

Observed food items on top cold holding unit in back area to be overfilled and spilling onto other food items. Please store food items up to fill line in container. Corrected on site.

Please lower temperature of unit to make sure food items are kept at 41*F or below. Food items were moved to walk-in and temperature was lowered to below 41*F by end of inspection.
