Observed debris and splatter on walls and on flooring in hard to reach areas. Please clean floor and walls regularly.


Observed scoops in dry goods bins with scoop handle touching food items. Please store scoops in a way to avoid handle touching food.

Facility needs to have a regular cleaning and deep cleaning schedule to avoid any build-up of debris and dust along equipment. A re-inspection will be conducted within 30 days to make sure facility has improved its cleaning and organization practices. If issues and repeat violations continue a Yellow Placard or Red Placard may be issued along with re-inspection fees.

REPEAT VIOLATION:
Observed ice scoop being stored on plate on top of ice machine, plate had food debris and dust. Please make sure scoop is stored in a clean container.

Observed scoops in dry good bins to have food debris on handles, please store scoops in manner to avoid food debris build-up on handles.

Observed food items in small reach-in cooler without lids, please make sure items are covered to avoid any cross contamination.

Observed items in walk-in cooler without lid, please cover items to avoid cross contamination.

Observed a a metal try holding salad to be overfilled above food line, please keep food items below food line to avoid contamination.

Observed opened jars of mustard in dry storage area, container states that jar needs to be refrigerated after opening. Please discard jars.

Observed dented can of sauerkraut in dry storage area, please discard can.

Observed bag of rice on floor near back area hand sink, please store food items at least 6 inches above ground.


Observed paper towels dispenser not able to dispense paper towels due to ran out of batteries. Batteries were replaced. Please make sure employees are washing their hands and paper towel dispensers are working properly.


Observed cans of unopened drinks meant for employees in small reach-in cooler across from cook line, please store employee drink and food in a designated area away from facility food items. Chef stated they do have a separate area and will move drinks to this area.


Observed sanitizer dispenser at three comp sink to be above 200 ppm, please make sure sanitizer is at 200ppm quat.

Observed sanitizer bucket below 200 ppm, please make sure sanitizer is at 200 ppm quat.

Observed test strips to have water damage, please replace test strips.


REPEAT VIOLATION:
Observed no Food Safety Manager Certificate, please have employee take course and post certificate for inspection.


Observed a spray bottle of degreaser with lid not properly secured, please make sure all spray bottles and containers are propelry sealed and closed.


Observed dust build-up along back wall near air curtain, please clean wall and air curtain regularly to avoid dust build-up.

Observed food debris in hard to reach areas under equipment along cook line, prep line, dry storage room and dish washer area.

Observed pre-packaged beef wrapped in plastic on bottom rack in walk-in cooler to be dripping and blood soaked cloth under rack. Please place any meat items in metal trays to avoid dripping and spills onto floor.


Observed mold and food debris on gaskets of small reach-in cooler across from cook line, please make sure equipment is cleaned regularly.

Observed dust and debris on top of dish washer, please clean dishwasher regularly.

Observed dust build-up along fan guards of walk-in cooler, please clean fan guards.

Observed dust on clean utensils and equipment due to dust build-up along back wall near back door. Please monitor equipment and clean as needed if it has dust build-up.

Observed mold along seal/caulking of rinse sink near dish washer, please clean regularly to avoid mold build-up.
