Observed paper towels stored out of dispenser. Per owner, lost key to dispenser. Ensure paper towels can be stocked in dispenser.

Observed a whole raw chicken (in original packaging) thawing at room temp on shelving in dry storage area. Use only one of four approved thawing methods:
1) Inside refrigeration
2) As part of the cooking process
3) Under cool running water (70° F) for no longer than two hours
4) In a microwave, provided food is immediately prepared after thawing.

Chicken was still below 41*F at time of inspection, and moved to refrigerator. CORRECTED ON SITE.

Observed unlabeled spray bottles, and spray bottles with other chemicals stored in container. Ensure all spray bottles are labeled with contents.

Observed grease accumulation on hood filters. Clean.

Inspection Memo:


First routine inspection, Green Placard issued, as all Major Violations were able to be mitigated during inspection. Several alternate refrigerators were turned on for facility to have adequate short term refrigeration until larger refrigerators repaired.

Reinspection of broken refrigerators required. Potentially hazardous food (PHF) may NOT be stored in broken refrigerators until refrigerators are repaired. Inspector to return on/around 8/29/24 for reinspection. Facility must show adequate amount of long term refrigeration (at least one large 3 door refrigerator working), and broken units removed from facility. Failure to comply may result in a Yellow Placard or reinspection fee of $228 per hour.
08/27/2024: Routine Inspection

Observed no chlorine test strips on site for sanitizer. Provide test strips.

Observed thermometer on large reach in refrigerator to be inaccurate, and smaller prep refrigerator did not have thermometer. Ensure all refrigerators have accurate thermometers.

1. MAJOR VIOLATION - Observed large reach in refrigerator holding food at 57*F. All potentially hazardous food must be held at or under 41*F. PHF that was out of temperature control for more than 4 hours was voluntarily discarded (two small bowls of soup, one (5) gallon bucket of yogurt, two large bowls of rice, and two pints of whipping cream). Several items, per owner, were just cooked one hour previously. These were transfered to another refrigerator, or the freezer. Reinspection required. MITIGATED ON SITE.

2. MAJOR VIOLATION - Observed small prep top refrigerator in front prep area holding food at 58*F. Per owner, all potentially hazardous food in unit was made approximately 1 hour earlier, or opened from a can one hour previously (dolmas). All PHF transferred to alternate refrigeration. Reinspection required. MITIGATED ON SITE.

1. Observed clean utensils stored in (clean) bucket on floor. Store all utensils at least 6" off of the ground. CORRECTED ON SITE.
2. Observed unsecured pressurized cylinder. Secure ASAP.

Observed sanitizer solution in 3 compartment sink at over 200ppm chlorine (bleach). Maintain at 100ppm. CORRECTED ON SITE.

Inspection Memo:


On-site for follow-up of Major Violations noted in routine inspection report dated 3/8/24.

Observations:
1) Hot water available at >120*F. In compliance.
2) Commercial pest control company provided service to facility on 3/12/24. Report available. Pest control company must provide weekly service to facility until cockroach issue is gone, and then can return to regular service schedule.
3) Observed facility cleaned behind/under cookline, refrigerators, 3-compartment sink, and grease trap.
4) Observed several dead cockroaches in kitchen area. This is to be expected based on how pest control products work/cockroach lifecycle. All cockroaches cleaned up during reinspection. Continue cleaning up dead cockroaches immediately.
5) Discussed with facility owner about keeping a strict cleaning schedule to help prevent future activity.

Green Placard issued. Facility permitted to reopen. Inspector to follow up with a verification inspection in approximately one month (on/about 4/10/24). Weekly pest control reports must be available, and no cockroaches evident (dead cockroaches must be cleaned up immediately). Failure to comply will result in a Yellow Placard and reinspection fee of $448.

Notes:
-Per routine inspection report dated 3/8/24, trash behind facility to be cleaned up by 3/18/24. Failure to comply will result in a Yellow Placard and reinspection fee of $448. Send inspector photo of dumpster area.
-No change of ownership/new permit application necessary. Facility is a franchise/under a new partnership, but permit holder remains the same.

03/13/2024: Revisit/Reinspection

Inspection Memo:



Facility is CLOSED due to lack of hot water and cockroach activity/lack of pest control reports. Reinspection required to reopen. Facility MUST do the following to reopen and avoid reinspection fees:
1) Hot water of at least 120*F must be available.
2) Pest control company must service facility; report with findings and recommendations must be available to inspector during reinspection; facility shall continue with weekly service until issue is resolved.
3) Conduct DEEP CLEAN of facility, including behind the cookline, behind all refrigerators, under the 3-compartment sink/dishwasher, and anywhere else where grime and grease accumulation occurs.
4) All cockroaches must be cleaned up.

Red Placard Posted. Call when ready for reinspection. If these items are not completed and a reinspection is performed, a reinspection fee of $448 will apply, and facility will not be permitted to reopen until the items above are completed.

*Note: Change of ownership permit application must be received and paid for by 3/18/24 and trash in back dumpster area must be cleaned up by 3/18/24. Failure to comply may result in a Yellow Placard and reinspection fee of $448.

03/08/2024: Routine Inspection

Observed grime accumulation on floors under cookline, under 3-compartment sink/on the wall behind 3-comp sink, and behind the grease trap. See violation #23. These areas must be cleaned up as part of the conditions of reopening. Conduct deep clean of facility.

MAJOR VIOLATION - Observed significant number of dead cockroaches in facility, including on the floor by the prep sink, behind the grill, behind refrigerators, and on the wall. Per manager, pest control serviced facility two weeks prior. However, no pest control report was available. No live cockroaches were observed.

Facility is CLOSED due to cockroach activity/lack of pest control reports. Reinspection required to reopen. Facility MUST do the following to reopen and avoid reinspection fees:
-Have pest control company service facility; report with findings and recommendations must be available to inspector during
reinspection; facility shall continue with weekly service until issue is resolved.

MAJOR VIOLATION - Observed water temperature at 65*F. Per manager, the hot water heater broke earlier in the day and a service
technician has been contacted. Hot water must be supplied at a minimum of 120*F.

Facility is CLOSED due to lack of hot water. Call for re-inspection.

1. REAPEAT VIOLATION - 3RD CITATION - Observed significant trash accumulation in the back dumpster area. Clean up all trash. Reinspection will occur on/about 3/18/24. Failure to comply may result in a Yellow Placard and reinspection fee.


2. Observed household pest control products in facility. Household pest control products are not permitted in food facilities; facility must be serviced by a professional pest control company. Remove products from facility.

Facility does not have a current Certified Food Safety Manager. Food facilities that prepare potentially hazardous foods must have, at minimum, one Certified Food Safety Manager on staff. Obtain current Food Safety Manager Certification ASAP.

Observed raw chicken thawing at room temp on the floor in cardboard boxes. Use only one of four approved thawing methods:

Raw chicken was probed and found to be less than 41*F (parts were still frozen, max probed temperature of 35*F). Raw chicken was placed in refrigerator. CORRECTED ON SITE.

Observed pressurized cylinder attached to soda dispenser by the hose, but leaning at an angle. Secure pressurized cylinder with a chain or rope to a solid structure.

2. Observed grime buildup on refrigerator handles near front counter, and handles under grill. Clean.
3. Observed grease accumulation on hood filters. Clean.

Observed sanitizer bucket in front prep area at 0ppm chlorine and sanitizer bucket in kitchen area at 200ppm chlorine. Maintain sanitizer solution at 100ppm chlorine, use test strips to ensure correct concentration. CORRECTED ON SITE.

1) Observed cigarette butts on ground on side of facility. Clean.

2) Observed bags of trash on ground in dumpster area. Per manager, this garbage is not from the facility, but dumped there by the public, and is placed in the dumpster every evening. Ensure all trash around facility is picked up and facility premises maintained clean and free of trash.

1) Observed grime accumulation on refrigerator handles in front prep area. Clean. CORRECTED ON SITE.

2) Observed significant grease accumulation on hood filters. Clean.

3) Observed grease accumulation on side of deep fryer. Clean.

Observed paper towels out of dispenser at front hand wash sink. Current dispenser is for a different shape of paper towel. Order correct paper towels for dispenser or provide dispenser that fits the paper towels on site.

All employees must have a valid CA food handler card. Certificates must be maintained on site and made available upon request. Comply by 12.21.2022


Observed trash and cigarette butts on the ground around the facility and in the parking lot. Maintain premise clean and free of litter.

Observed fly in back prep area. Facility must be free of insects and pests.

Observed red sanitizer bucket used to thaw chicken in the 3-compartment sink. Do not do this.. Only use red bucket for sanitizer to prevent chemical contamination to food. Use prep sink to thaw food, wash vegetables, etc.

Observed grease accumulation on the hood filters. Clean.

Observed broken faucet at the prep sink. Repaired at the time of inspection.

Observed no paper towels provided at the front handwashing sink. Observed no soap provided at the back handwashing sink. Handsinks must be supplied with soap and paper towels at all times. Corrected on site.

Observed excessive trash piled up behind the facility. No garbage dumpster available to store refuse. Facility must have trash pick and an appropriate dumpster or trash can to store refuse. Trash must be stored in a closed and secure receptacle that prevents the attraction of pests. Correct immediately.

Provide chlorine test strips and use to verify sanitizer is prepared to 100ppm chlorine.

Observed grime and food residual on handles of refrigeration units. Clean.

Observed no sanitizer prepared or available in the facility. Facility must have a sanitizer solution with 100ppm chlorine prepared and available to sanitize food contact surfaces at all times. Note: chlorine sanitizer obtained at the time of inspection.

Observed grime accumulation on the floor under the racks, refrigeration equipment, 3-comp sink and prep sink. Pull out equipment and conduct a deep clean.

Repeat violation - observed grime and food residual on handles and surfaces of refrigeration equipment throughout the facility. Thoroughly clean handles and wash hands frequently to prevent cross contamination from equipment to hands. Comply by 4.27.2022.

Observed inoperable warewasher. All equipment must be fully operational. Warewasher must produce a 50-100ppm chlorine sanitizer residual on the dish surfaces. Contact technician to service equipment. Warewasher shall not be used until repaired and a verification inspection has been conducted by PCEH. Comply by 4.27.2022

Observed fly on equipment on the main cookline. Facility must be kept clean and free of pests.

Repeat violation - 4th citation - Food Safety Manager Certification not available at the time of inspection. Facility must have, at minimum, one employee with a valid food safety manager certification. Certificate must be maintained on site at the facility and available for review upon request.
