REPEAT VIOLATION Ice bags must be labeled with facility name and address. Provide label/ stickers with this information to be stuck onto bags until bags with facility address and name can be ordered. Comply by 4/2025 or violation will become a Major Violation.

Observed floor drain under soda syrup boxes with plastic tubing draining without 1 inch air gap. (see photo) Cut back plastic tubing so that it drains with a one inch air gap.

Observed food prep occurring with sanitizer bucket at 0 ppm. Employee made a new sanitizer bucket at proper concentration. Corrected on site. Routinely check sanitizer concentration to ensure it is at proper concentration.

Observed sticky residue on underside of soda fountain nozzles. Routinely clean and sanitize this area.

Observed ice machine with inside baffle accumulating biofilm. (see photo) Drain ice machine of all ice, clean, and sanitize all inside surfaces with special attention to underneath baffle. Routinely clean ice machine.
