Repeat Violation - Observed duct tape on the door handle of the unit (SYS 3H-2) below the sushi make table. Duct tape is not approved for use in a food facility as it is not sanitary or washable. Remove duct tape and repair handle.

Observed a build-up of debris in the tracks of the sliding doors below the sushi make table. Please clean.

Facility has a HACCP plan for acidified sushi rice but has not been keeping written pH logs since Sept. 2024. The HACCP plan must be followed and written records of the pH for each batch of rice must be maintained and available for review. Correct immediately.
