Observed some grime build-up on floors throughout kitchen.
Floors are to be kept clean and well maintained.
Correct by 11/10/2021.

Observed sanitizer at front waiter station at 0ppm Quat.
Sanitizer shall be prepared at 200ppm Quat at the beginning of the day and switched out every 4 hours or sooner as needed.
Corrected on site.

• Observed barriers installed at point of sale.
• Per Plan, high touch areas are sanitized every hour to hour and a half.
• Facility has reduced the seating capacity of the onsite dining areas to provide a minimum of 6 foot physical distancing.
• Observed hand sanitizer provided at entrance to facility and at each table.
• Menus are being sanitized after each use.
• Facility has not prepared a Workplace Specific Plan. Onsite dining areas shall be closed until facility has prepared and implemented a Workplace Specific Plan. A Workplace Specific Plan template was emailed to operator at time of inspection.
• Employees will be trained on contents of plan and training will be documented.
