Repeat Violation - Observed raw items on the menu that are not marked or disclosed as having raw or less than thoroughly cooked ingredients (i.e. steaks, ahi salad, ahi tacos). Each applicable item must be marked with an asterisks (or equivalent symbol) that corresponds to a written disclosure and advisory statement.

Observed pooling water and grime accumulation the sinks in the bar. Conduct a deep clean.

Observed water leaking from the water pipes under the 3-comp sink in the bar. Repair by 3.11.2021

Observed broken drainage pipe under the handsink in the bar. Repair. Comply by 3.11.2021

Observed uncovered food in the walkin cooler and reachin refrigerator. Cover food to prevent contamination.

Observed sauces in hot holding unit probed at 70-80*F. Foods that are reheated for hot holding must be rapidly heated to 165*F and then held at 135*F. Remove from foods from hot holding unit and and rapidly heat to 165*F on the stove/griddle. Corrected on site.

Observed employee beverage in uncovered mason jar on the salad prep refrigerator. Employees beverages must be a closed container (ie. lid and straw).

Observed mold accumulation on the racks in the reach in refrigerator in the bar. Deep clean.

Observed significant dust accumulation on the side on the oven. Clean.

Observed grime and debris accumulation in the floor sinks in the bar. Clean.

Observed significant damage to the floor in the event center walkin cooler. Observed large missing pieces of vinyl and exposed bare wood. Repair/replace.

Observed jammed/inoperable paper towel dispenser in the employee restroom. Properly load paper towels so they dispense properly. Correct immediately.

Facility does not have an employee with a current food safety manager certification. Facility must have, at minimum, on employee with a current food safety manager certification. Obtain valid certificate and maintain on site at the facility. Comply by 4.25.2021

Observed grime accumulation on the can opener. Clean and sanitize.

Observed employee prepping burger garnish next to the fryer/cookline with in no sanitizer bucket prepared. A sanitizer solution must be provided at all times to sanitize food contact surfaces. Corrected on site.

Observed roll of paper towels on counter next to the handsink. Paper towels must be provided in a dispenser. Correct immediately.

Observed several unchained CO2 and propane cylinders outside the kitchen back door. Gas cylinders must be securely fasten/chained to the wall or equivalent structure. Correct immediately.

Observed grime and grime accumulation on the wall and ceiling in the kitchen especially around vents. Conduct a deep clean! Comply by 1.9.2020
