1.) Employee with Food Safety Manager Certificate on file with EH is no longer employed at facility. Acquire new certification within 60 days, 9/19/2021. Please provide a copy to Jason Gaitan at jasongaitan@placer.ca.gov.

1.) Observed refuge and returnables food stored on floor of walk-in cooler. Please provide a nonabsorbent, durable, cleanable, leak proof, and rodent proof containers to store returned food.

2.) Observed returned food store in 3-comp sink. Please provide a nonabsorbent, durable, cleanable, leak proof, and rodent proof container(s) for returned food.

1.) Observed no sanitizer bucket in the facility. Provide a sanitizer bucket at 100ppm chlorine 200ppm quaternary ammonia.

2.) Observed no sanitizer test strips available at this facility. This facility is currently utilizing chlorine. Please get chlorine test strips ASAP.
Discussed with manager about chlorine to water ratio.

- Please clean dust/mold on fans inside walk in cooler.

• Observed customers self served with soda without precautions/customers were not wearing face covering; touch containers with bare hands.
Please close self service soda machine and modify delivery of soda by providing to guests individually.

Observed hand sink not provided with soap in dispensor. Per employees they do not wash hands at hand sink in saleas area. Hand sink must be supplied & accessible for employees to wash their hands at all times. Comply by 9/1/2020.


Observed no sanitizer provided for surface cleaning. Provide 200 ppm quat sanitizer or 100 ppm bleach at all times. Corrected on site.

Provide Food Safety Manager Certification ASAP (has expired 0n 6/22/2020).


Observed hot holding at upper part of the warmer probed at 128*F. Adjust temperature at or >135*F. Comply by 9/1/2020.
