REPEAT VIOLATION - Observed 1" gap under both front and rear screen doors. Provide weather stripping to prevent vermin entry.

MAJOR VIOLATION - Observed inoperable spray bottle with less than 1/2" of bleach solution, and no other sanitizer on site. Sanitizer must always be available in the food facility, and a sanitizer solution at the proper concentration must be set up at the start of each day, and changed as often as needed to maintain proper concentration (100ppm bleach). Sanitizer brought to facility during inspection. CORRECTED ON SITE.

REPEAT VIOLATION - Observed no thermometer in refrigerator in main food prep area, or in refrigerator holding milk. Provide a thermometer in all refrigerators that contain potentially hazardous food. Note: Thermometers ordered during inspection.

MAJOR VIOLATION - Observed no paper towels at handwash sink. Handwash sink must be supplied and accessible at all times. Paper towels restocked in dispenser during inspection. CORRECTED ON SITE.

REPEAT VIOLATION - Observed no utensil in consumer self service area for donuts. Provide.

Observed reach in refrigerator in prep area with no thermometer. A thermometer must be provided in every refrigerator that hold potentially hazardous food (PHF) to monitor temperature and ensure food is maintained at or below 41*F. Provide thermometers in all refrigerators that hold PHF.

1. Observed 1" gap under back door. Install weather stripping to prevent vermin entry.

2. Observed employee items stored with facility food items. Keep personal items/food separated from facility food. CORRECTED ON SITE.

MAJOR VIOLATION - Observed small reach in refrigerator in food prep area holding an ambient temperature of 60*F. Probed cheese at 60*F. All potentially hazardous food (PHF) stored in the refrigerator for over 4 hours was voluntarily discarded. Refrigerator turned down at time of inspection and observed holding an ambient temperature of 41*F. CORRECTED ON SITE.

Observed handwash sink partially blocked with dirty dishes. Handwashing sinks are for handwashing ONLY and must be supplied and accessible at all times. Dishes removed from handwashing sink. CORRECTED ON SITE.

Repeat violation - Food safety manager certificate expired. Obtain valid certificate and maintain on site at the facility.

Observed empty paper towel dispenser in the restroom. Maintain supplied at all times. Corrected on site.

Observed two inoperable refrigeration units outside facility. Remove from premises.

Observed floor leak in the back storage area. All food and equipment must be kept 6 inches above the floor to prevent contamination. Repair leak.
