1.) Observed nozzle for soda dispenser (horchata) with significant grime build up. Please maintain all soda dispenser nozzles to prevent grime build up.

1.) Observed caulking for 3-comp sink in disrepair. Please repair/replace caulking for 3-comp sink ASAP.

1.) Observed no sanitizer spray bottle in food prep area. Provide a sanitizer bucket at 200ppm quaternary ammonia.

1.) Observed fluorescent light bulb in walk-in cooler with no shatter proof container. Please provide all fluorescent lights with a shatter proof container.

1.) Observed hole on wall for mop sink room. Please repair any holes on walls in this facility ASAP.

2.) Observed significant accumulation of trash and food debris underneath slushie machine. Please maintain floors to prevent accumulation of trash and food debris.

3.) Observed accumulation of red syrup accumulating on floor underneath soda dispensing area. Please maintain floors to prevent accumulation of syrup.

1.) Observed various containers of chili sauces, milk, and soda syrup boxes stored directly on the floor. Please keep all food items on at a minimum of 6-inches off the floor.

Observed janitorial sink hot water reaching 96F. Maintain hot water at service sink at minimum of 120F.

Covid-19 Guidelines

- Observed no covid-19 signage posted at the front entrance. Please post attached signage.
- Physical barrier installed at point of sale.
- Hand sanitizer available for employee and customers.
- Observed employee not wearing face coverings. There was only 2 employee onsite. Employee should wear face covering when they are unable to maintain 6ft distancing.
- Observed self-service soda fountain opened to customers. Disposable gloves were provided at the soda fountain for customer use. Observed customers not using the gloves provided while dispensing soda from the soda fountain and touching clean lids with their bare hands. Recommend relocating the lids behind counter and clean and sanitize all the high touch area more frequently.

06/16/2020: Routine Inspection

Observed manager food safety certificate expired. Provide valid manager food safety certificate.

Observed heavy soda spill under the soda box area and debris under the racks in the walk in cooler. Maintain floors clean.

Inspection Memo:

This is a re-inspection to follow up on violations noted on 10/10/19.

Observation:

7)A) Prepackaged hamburger in the reach in refrigerator measured at 48F. The ambient temperature on the bottom level of this refrigerator is at 45F-50F and the top level is at 41F.
B) A container of pico de galo on the customer self-service area measured at 54F. The ambient temperature in the customer self-service refrigerator and cold top was measured at 50F. Manager voluntarily discarded the container of pico de galo on site.

Repair these unit and make sure it's cold holding at 41F or below before using it to store potentially hazardous food.

14) Observed no sanitizer bucket in the food prep area. Also observed employee using glass cleaner to wipe the food prep counter. Instructed manager to make chlorine sanitizer on site and re-clean the food prep counter. Must provide chlorine sanitizer strip immediately.

34) Repair caulking on restroom hand sink and behind 3 comp sink.

41) Provide back flow preventer/ hose bib on mop faucet.

45c) Repair vinyl cove base in the warwashing area under electrical box and in soda box area next to the restroom. Repair by next routine inspection.

A 2nd chargeable re-inspection of $388.00 will be conducted 10/22/19. Failure to correct violation may result in a yellow placard and/ or chargeable re-inspection.

10/10/2019: Routine Inspection

A) Observed caulking on the restroom hand sink in disrepair and the restroom in need of cleaning. Repair caulking on restroom hand sink and clean the restroom.
B) Observed toilet paper on top of the dispenser. Keep toilet paper rolls in the dispenser.

Observed the following potentially hazardous food out of temperature:
- One open bag of taquitos and 4 packs of unopen taquitos on the counter measured at 74F-81F. Manager opened the 4 packs of taquitos and discarded it on site along with the opened bag of taquitos.
- Prepackaged burritos and hamburgers in the reach in refrigerator measured at 45F-48F. There was also a container of pico de gayo on the customer self services counter measured at 48F.
Manager adjusted the temperature to 41F.

Maintain all potentially hazardous food cold holding at <41F or hot holding at >135F.

Observed light on the far end of the walk in cooler not working. Repair.

Observed no backflow preventer on the mop faucet. Provide a hose bib on the mop faucet to prevent sewage from backing up into the potable water.

Observed caulkings behind the 3 comp sink in disrepair. Repair.

A) Observed spills and build ups on the floors through out the warewashing area, back storage by the restroom and walk in cooler. Keep the floors clean.

B) Observed soda spray on the wall by the soda box rack in the warewashing and back storage by the restroom. Keep walls clean.

C) Observed vinyl cove base under the electrical box in the warewashing area and by the back storage soda box rack peeling off the wall. Repair by 04/10/2020.

Observed cluster of food and non food items stored in the janitorial area. Clean and organize this room.

Observed no sanitizer buckets in the food service area. Provide.
Repeat violation.

Observed lots of soda syrup boxes stored on the floor in warewashing area and the soda box rack by the restroom. Store all food and single use articles at least 6 inches of the floor.

Observed wall mounted soap dispenser broken. Repair/replace.
