Observed several unlabeled spray bottles with unknown liquid contents. All working containers of cleaning agents, sanitizer, water, etc. must be clearly labeled with the common name of its contents. Correct immediately.

Observed a few dead cockroaches in several locations in the downstairs storage area including the flour room. Observed dead cockroaches in several sticky traps in the downstairs storage area. Clean up dead insects, and removed old sticky traps and replace with new trap. Note: be sure to indicate on the trap the date it was placed to better track current insect activity.

Continue to work with integrated pest control service to prevent vermin issue and maintain records/reports of pest control service dates and the services provided. Any notes or recommendations from pest control regarding sanitation or vermin proofing must be addressed.

Observed grime and debris accumulation on both can openers in the many prep area. Thoroughly clean and sanitize to prevent cross-contamination.

Observed several employee personal items (ie. cellphones, purse, beverages, food) stored directly adjacent to, and encroaching into the food prep area next to the door to the dining area with the piano. Employee personal items must be stored in a designated area that is completely separated from the food prep area to prevent cross-contamination. Corrected on site.

Observed debris accumulation on the shelves under the prep counter across from the boiler/cookline. Clean.

Observed used wiping clothes lying on prep area. Wiping clothes must be kept submerged in a bucket of sanitizer with a 200 ppm concentration of quat sanitizer.

Observed Decon rodent poison under desk in cafe office. Home pest control products are not permitted inside food facilities. Remove Decon product immediately and contact professional pest control operator to mitigate rodent issue.

Observed several dead cockroaches in multiple locations throughout the facility, including: on the side on the walkin freezer next to the bakery, behind ovens in the bakery, flour storage room, in the downstairs freezer storage area, janitorial storage area, "old bakery area downstairs"

Observed rodent droppings in the downstairs walkin in storage area.

Facility must be maintained vermin proof and free of pest and insect. Contact professional pest control operator to mitigate vermin issue. Comply by 9.30.2020

Observed mechanical warewasher produce 0ppm chlorine residual. Note: warewasher is hooked up to chemicals but can also be operated as a high temperature sanitizing warewasher. Temperature test strip was run through washer and surface temperature on utensils and equipment is reaching 160*F. Testing equipment and materials shall be provided to adequately measure the applicable sanitation method used during manual or mechanical warewashing. If using temperature as a method of sanitation, provide temperature test strips.

Observed several bags food waste, trash, and recyclables piled on the floor in the downstairs refuse accumulation area. Refuse storage area in not vermin proof. Observed doors with half inch mesh and gaps under the door. Per cafe manager, dumpsters are located on opposite side of Weimar Institute premises and trash is transported by facility grounds staff once per day.

All refuse, recyclables, and returnables shall be kept in nonabsorbent, durable, cleanable, leakproof, and rodentproof containers and shall be contained so as to minimize odor and insect development by covering with close-fitting lids or placement in a disposable bag that is impervious to moisture and then sealed.

Refuse, recyclables, and returnables shall be removed from the premises at a frequency that will minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents.

Observed leak under prep sink. Repair.
