Observed: Utensil racks and containers are dirty. Mold in ice machine.

Corrective Action: Utensil racks and containers shall be cleaned and sanitized to remove old food particles. Clean and sanitize ice machine to remove mold.

Observed: Hand washing sink hot side valve is broken. Hand washing sink is dirty with food grime.

Corrective Action: Hand washing sink shall be repaired, cleaned, and sanitized.

Observed: Uncovered container of beans in walk in cooler not in the act of cooling.

Corrective Action: Food items shall be covered at all times unless being actively cooled. Item was corrected on site.

Observed: Employee eating food while standing over the dish pit sink.

Corrective Action: Employees shall eat their food in designated areas away from food preparation.

Observed: Facility using plastic water bags hanging from the ceiling as some form of fly deterrent. UV bug zapper placement over kitchen door is no longer acceptable due to potential of dead bug parts falling in refrigerator door when open.

Corrective Action: Plastic water bags were removed by manager, corrected on site. UV bug zapper shall be moved to location in kitchen that does not have the potential from dead bug parts to fall in food prep areas.

Observed: Two (5) gallon buckets of cooked potatoes in walk in cooler with no cover. Bowl of unidentified sauce covered with a plate in walk in cooler.

Corrective Action: All food shall be covered at all times unless being actively cooled. Facility shall stop using plates as covers, and use plastic wrap or proper container cover.

Observed: Small amount of mold in ice machine. HVAC registers above hot line have dust on them.

Corrective Action: Wash and sanitize ice machine to remove mold. Clean HVAC registers above hot line to avoid potential food contamination.

Observed: Prep refrigeration unit on cook line temped at 52F.

Corrective Action: Refrigerator shall be put out of service until it can maintain <41F. Potentially hazardous foods were voluntarily discarded.

Corrective Action: Replace plastic catchment or relocate bug zapper so dead bugs do not fall into refrigeration unit when door is open.

Observed: Small amount of mold in the ice machine.

Corrective Action: Clean and sanitize ice machine to remove mold.

Observed: Bowls being used as a scopp for dry goods. Several uncovered item in prep refrigeration unit.

Corrective Action: Facility shall use a handled scoop for dry goods. All items shall be covered at all times unless being actively cooled.

The use of printable pictured items to encourage guest to follow guidelines. Easier to read than small print on sandwich board.

Observation: Found fruit bowls in refrigeration unit with no covers.

Corrective Action: All food shall have a cover while being stored in refrigeration units.

Observation: Found dirt and old food debris under equipment and on floors throughout facility. Observed signs of wear on floor coping beneath ware washing area.

Corrective Action: Clean floors throughout facility to remove dirt and food debris in facility. Repair/replace coping section in ware washing area of kitchen that is starting to crack.
