REPEAT VIOLATION - Observed floor tiles to be torn and missing in sections throughout the facility. Floors are significantly more damaged and now pose a tripping hazard to customers.

Floors shall be smooth, nonabsorbent and easily cleanable. Water and debris can build up in cracks and crevices causing bacteria and mold to grow.

Repair/replace flooring with approved flooring material. Go to Placer Co. Env. Health (online or in person) for a list of approved flooring materials and a plan review application.

Observed candy spilled on the shelves in the candy aisle. Please remove baskets/shelving and thoroughly clean all spills.

Observed handle of small two door freezer near registers to be taped on to the door. All equipment shall be maintained in good condition. Please repair handle properly.

Observed paper towels on a wooden dowel in the restroom. Paper towels must be provided in dispensers at all times to prevent potential contamination. Correct immediately.

Observed sticky trap in the back storeroom with deceased cockroaches. No live pests were seen on this day. Per manager, the facility does have a licensed pest control operator come on a regular basis.
Continue with pest control service (increase frequency if necessary) and follow all recommendations provided by pest control company.
