Observed sliced carrot stored inside refrigerator by the cook line using used plastic bag. Use clean and sanitized container at all times.
Contaminated carrot in used plastic bag is discarded.


Observed no sanitizer solution provided for surface cleaning. Provide 100 ppm chlorine or 200 ppm quat sanitizer at all times. Corrected on site (100 ppm bleach solution provided).


Observed accumulation of dirt and dust on the roof around light bulb. Clean dust/dirt and cover light bulb ASAP.

Observed dirt and grime on mop sink and equipment surfaces in ware washing area. Deep clean.

Clean fans inside walk-in cooler and all vents inside the facility.


Observed 3 comp sink provided with hot water at 110*F. Three comp sink must be provided with hot water at a minimum temperature of 120*F. Corrected on site.


Observed egg stored on the middle shelf inside walk in cooler. Egg must be stored at bottom shelf. Corrected on site.


Provide soap in dispenser ASAP.


Replace chopped plastic cutting board ASAP.
