Observations:

- observed vegetable refrigeration temperature for most displayed food items probed <41*F .
- Observed cold holding for noodle probed 45*F and for broccoli 44*F. Adjust temperature at or below 43*F.
- Observed dirt build up on faucet at both hand sinks. Deep c lean.
- Observed floor around food serving area coved with unclean carpet. Stop using carpet in the facility. Second violation.

Reminder:

Please submit plan review application and specification sheet for the newly installed reach in freezer (equipment only) with in 72 hours. Second notice.

This is combined routine inspection and complaint response report.
Alleged complaint: Dirt inside the restaurant
Discussed the complaint with the manager Andrew Stephens. Per manager, he is ready to correct the complaint

Observations:
- Observed dirt build up on carpets around food self-serving area. Remove carpet from food serving area; carpet is not allowed inside a food facility. Please keep cleaning floor frequently.
- Observed sauce spill and food droppings on food waiting table. Clean frequently.
- Observed grime build up below wok, dish washer, food warming table and food display refrigerator unit. Deep clean.

Observed cold holding in refrigerator units for sliced vegetables and diced tomato probed 50*F. Cold holding must be adjusted at 41*F or below. Comply by 5/13/2021. Re-inspection will be conducted on 5/17/2021. Second violation.


Observed dish wash machine produced 0 ppm residual chlorine. Dish washer mus produce 50 ppm chlorine residue at all times. Corrected.
