Observed the back door to be propped open at beginning of inspection. After inspector brought it to the attention of manager, they closed it. Ensure door is closed unless trash or cleaning is actively being done, to prevent pest entry into facility.

MAJOR VIOLATION: Observed the dish machine to be dispensing chlorine at 0PPM. Chlorine shall be dispensed between 50-100PPM. Inspector checked machine twice. Do not use dish machine again until fixed. Send photo of correction to your inspector. For now, use 3 compartment sink to wash dishes manually.

Observed liquid waste discharge pipes for the walk-in cooler and another liquid waste discharge pipe to be sitting below the flood plane of the floor sink. Provide air gap above flood plane, minimum 1 inch.

Observed the small cooler for pickles, cheese, sliced tomatoes to be holding temp at 44*F. Probed cheese at 47*F, pickles at 42*F. Turn down temp of unit. Do not use unit again to store potentially hazardous foods until it is holding temp at or below 41*F.

Observed the liquid waste discharge pipe of the walk-in cooler to be sitting ~3 inches below the flood plane of the floor sink into which it drains (below prep sink). Provide an air gap for this liquid discharge pipe a minimum of 1 inch above the flood plane of the floor sink.

REPEAT VIOLATION: Observed cooler in front of house used for potato salad, macaroni salad, and ranch to not have an internal free standing thermometer. Provide internal free standing thermometer so that employees may easily verify that unit is holding temp at or below 41*F. Manager provided thermometer. CORRECTED.

Observed potatoes being prep at prep sink next to hand wash sink. Provide a splash guard between hand wash sink and prep table so as to prevent contamination of foods prepped in this area and in close vicinity to splash from hand wash sink.

Observed hand wash sink in front of house to have non functioning cold water handle. Repair so as to facilitate hand washing. Hand wash sink shall be provided with hot water between 100-108*F. Because only hot water is working at hand wash sink, temperature is currently >108*F.

Observed milk wash to be at 49*F. Milk wash was being held on ice and not sufficiently immersed in ice. Cold PHF foods shall be held at or below 41*F. Employee corrected by adding more ice. Final temp taken was 41*F. CORRECTED.

Observed hand soap dispenser of hand wash sink in BOH to not be functioning. Manager corrected. CORRECTED. Provide enclosed single-use paper towel dispenser for hand wash sink in front of house.

If pest activity is observed in restaurant immediately consult pest control.

Observed upright reach-in cooler across from ice machine to have leaking condenser which was leaking into a plastic container. Please service/repair condenser to ensure it no longer leaks.

Advised new owner to submit an application for change of ownership and proof of payment for 2022-20223 health permit to PCEH.
