Routine Inspection Observations:
Hot water from 3 compartment sink observed at 120*F.
Warm water from hand sinks observed at 100*F. Hand sinks stocked with soap and paper towels.
Cold holding of potentially hazardous foods observed at <41*F.
No hot holding observed at time of inspection. Facility does not have hot holding equipment.
Chlorine dish machine verified at 50ppm.
Food protection manager and food handler cards verified.
07/08/2021: Routine Inspection

Clean the floors in the corner opposite the mop sink, as well as below all refrigerators in the kitchen to eliminate debris. Inspector could not tell whether some debris were rodent droppings. Facility receives monthly pest control service from Zap. After cleaning floors, monitor for rodent activity and treat accordingly.

Corrected on site.

At time of inspection, no sanitizer solution was prepared to store soiled wiping cloths. Owner corrected during inspection.

Observed several white plastic yogurt containers being reused to store meats, sauces, etc. throughout facility refrigerators. Phase out the use of single use plastic yogurt containers by purchasing multi-use commercial containers instead.

Observed leaking from the right compartment drain line at the 3 compartment sink. Repair all plumbing to be fully functional and without leaks.

Obtain chlorine test strips to check the sanitizer concentration in the dish machine.

Observed several food items stored on the cold top cutting board as well as nesting on foods inside of the cold top that, per the owner, are for the owner and not for customers. Owner states he does not like to throw away food and keeps it. Ensure personal food items are kept separate from food intended for customers. Store in a designated location.

Observed two bottles of water stored buried inside of ice within the ice machine. Discontinue storing food/beverage items inside of ice intended for consumption. Maintain ice free from sources of contamination at all times.

This is a re-inspection to follow up on violations noted on 7/30/2020.

All violation has been corrected except for the following:

34) Install a faucet at the 3 comp sink. Make sure the faucet is able to reach all 3 compartment. Correct by 1/30/2021. REPEAT VIOLATION.
39) Provide a probe thermometer that can read both cold and hot temperature. Correct immediately. Email picture as proof of correction to zxiong@placer.ca.gov by 8/14/2020.
44) Install self-closing device on back door. Correct by 1/30/2021. REPEAT VIOLATION.

COVID-19 Guidelines
• Facility has not prepared a COVID19 Workplace Specific Plan. Facility is currently doing take outs only. A copy of the COVID19 Workplace Specific Plan was emailed to the operator. Recommend completing a workplace plan even if indoor seatings are closed.
• Observed inconsistent use of face coverings by facility employee. Please follow California Department of Public Health (CDPH) Guidelines for face covering requirement. Per CDPH guidelines, people in California must wear face covering when they are engaged in work, whether at the workplace or performing work off-site; when interacting in-person with any member of the public; working in space visited by member of the public, regardless of whether anyone from the public is present at the time; working in any space where food is prepared or packaged for sale or distribution to others; working in or walking through common areas.
• No COVID-19 or physical distancing signs were posted in facility at the time of the inspection. Post COVID-19 signage. Copies of the signs was emailed to the operator.
• No physical barriers were provided at the point of sale and staff were not wearing face coverings. Staff should wear face coverings if physical barriers cannot be installed.
• Hand sanitizer was available at entrance to facility and by the register.
• Per owner, high touch areas are sanitized regularly.
• Employee training has not been conducted. Train employees on the Workplace Specific Plan. Document when the training is conducted and the names of employees in attendance.
• Laminated menus are still being provided to customers. Discontinue this practice or implement appropriate measures to sanitize menus between customer groups. Provide disposable or digital menus instead of laminated menus.

Reminders:
A re-inspection will be conducted on 8/5/2020. Failure to correct new and repeat violations may result in a yellow placard and/ or chargeable re-inspection.
07/30/2020: Routine Inspection

A) Observed raw meat stored above ready to eat food inside the 2 door reach in refrigerator. Must store raw food under ready to eat food. Correct by 8/5/20.

B) Observed a bag of opened flour stored directly on the floor in the dry storage area. Store food at least 6 inches off the floor.
REPEAT VIOLATION.

C) Observed containers of food nesting on top of other food items inside the 2 door reach in refrigerator. Discontinue this practice to prevent cross contamination.

Observed a probe thermometer that only reads at 50F and above. Provide a probe thermometer that can read below 50F. Correct by 8/5/20.

Observed spray nozzle at the 3 comp sink replaced with another spray nozzle without a faucet. Must install a faucet that can reach all 3 compartment. Correct by 1/30/2021.

Observed no sanitizer bucket for the soiled wiping cloth on the cutting board. Provide immediately.

Observed old food build up inside the sandwich prep unit. Maintain equipments clean. Correct by 8/5/20.

Observed clean stainless steel containers stored on the mop sink. Must food containers on clean surfaces.

Observed one container of cooked rice, chicken and onion gravy inside the 2 door reach in refrigerator that was cooked and cooled since yesterday 7/29/20 still at 49F to 50F. Manager voluntarily discarded these items on site.
Ensure food are properly cooled from 135F to 70F within 2hrs and from 70F-41F within 4hrs.

Observed 2 bottle of RAID home pest control spray bottle in the food prep area. Remove domestic pest control spray from the premises and hire commercial pest control company for any vermin problem. Correct by 8/5/20.
REPEAT VIOLATION.

Observed raw meat inside the 2 door reach in refrigerator at 47F-48F. The ambient temperature in this unit is at 46F-50F.
Owner relocated all potentially hazardous food (PHF) in this unit into the 2 door reach in freezer.
Repair this unit and make sure it is at <41F before storing PHF. Correct by 8/5/20.

Observed floor tiles under the front hand sink, mop area, 3 comp sink, ice machine and grease interceptor replaced with like for like.
Owner also replaced all the coved base in the front of house and back of house with tile base that are not approved by PCEH. These tile base are not 3/8 inch radius coved and is not flushed with the finished floor. Upon a remodel or change of ownership, the tile base must be replaced with coved base tile that has a 3/8 inch radius curve, is flushed with the floor and is 4 inches high. Plan must be submitted to PCEH prior to any construction work.

Reminders:
- Failure to correct violation may result in a yellow placard and/ or chargeable re-inspection of $388.00.
- This facility does not have a prep sink and approved cove base throughout the kitchen. The flooring by the janitorial area is also not level with the rest of the kitchen floor. A prep sink must be install, rubber cove base must be replaced with an approved type and flooring by the janitorial area must be leveled with the rest of the finished floor upon a change of ownership or remodeled.
- This facility currently have an above ground grease interceptor installed under the 3 compartment sink. Upon a change of ownership or remodeled, the above ground grease interceptor must be relocated outside of the food and equipment handling area or replaced with a self-cleaning type such as a "Big Dipper".


This is also a complaint inspection to follow up on complaint number CO0023914.

Nature of complaint: Complainant alleges that employee does not wash hands after using the restroom and that the restroom and kitchen floors are filthy.

Observation:
Observed facility cleaned and employee properly washing hands. All hand sink were fully stock with hand soap and single use paper towel. There were some violations noted during the routine inspection that was not related to the complaint. See comments on above routine inspection for details.

Observed no drain covers on the floor sink under the 3 comp sink. Correct by 1/10/20.

Observed self-closing on the back door not working. Correct by 1/10/20.

A) Observed one bottle of Ortho home pest control spray in the back storage area and multiple RAID home pest control spray in the front cabinets. Multiple fly traps were also placed with facility food and clean single use articles in the front cabinets. Discontinue use of home pest control spray or traps and hire pest control company for any vermin problem.

B) Observed chemical spray bottles on the mop rack with no labels. Correct by 1/10/20.

Observed clean pots and pan stored on top of the mop sink. Must store clean pots and pan on clean dry location. Corrected on site.

Observed the spray nozzle on the 3 comp sink leaking and tape was observed wrapped around it to prevent it from leaking. Replace the spray nozzle with a faucet that can reach all 3 compartment. Correct by 1/17/20.

A) Observed floor tiles under the front hand sink, ice machine and around the grease interceptor no longer smooth, durable, nonabsorbent and cleanable surfaces. Repair the floor tiles by 6/7/2020.

B) Observed black dust build up on the ceiling panel by the exhaust hood. Clean the ceiling panels by 1/10/20.

Observed a container of raw chicken on top of the box of oil and a container of cooked rice on the prep table by the back door measured at 45F-47F. Must keep potentially hazardous food cold holding at <41F or hot holding at >135F.
Corrected on site.

Observed no probe thermometer on site. Correct immediately.

Observed no employee food handler cards on site. Provide immediately. New employees have 30 days from the date of employment to obtain a food handler card.

A) Observed food containers containing spices, flours, sauce, etc. with no labels. Correct immediately.

B) Observed prepackaged mango juice in the U Star 1 door refrigerator in the dinning area with no labels. Must provide proper label for all prepackaged items that are up for grab by customer. Labels must include common name of food, ingredients, weight, allergens, manufacture name.

A) Observed dust and grime build up on the racks by the sandwich prep unit and inside the sandwich prep unit. Clean equipment and non food contact surfaces by 1/10/20.

B) Observed single use yogurt containers reused to store different kinds of food. Discontinue this practice. Must provide approved food grade multi use containers to store food. Correct immediately.

A) Observed raw chicken stored above ready to eat food inside the Atosta 2 door refrigerator. Must store raw food under ready to eat food at all time. Correct immediately.

B) Observed a bag of onion and pan of oil stored directly on the kitchen floor. Must store food at least 6 inches above the floor. Corrected on site.
