Observed linen cloths being stored on prep surfaces when not in use. Linen cloths shall be stored inside an approved sanitizer solution. The sanitizer solution and linen cloths shall be changed every 2-4 hours.

Observed employee who was wearing gloves use a linen cloth to wipe down food contact surface and then immediately started prepping food. No changing of gloves or proper hand washing had occurred. Employees are required to wash their hands; -Before beginning work -Before handling food or clean equipment/Utensils -As often as necessary during food preparation to remove soil and contamination -When switching from working with raw to ready to eat foods, -After touching body parts -After using the restroom -Any time when contamination may occur. Employees shall be educated on proper hand washing.

Facility had been scheduled for a re inspection however upon arrival at facility, all doors were locked.
Called the facility site number twice but no response.
Went around the drive thru to have an employee open the door for the inspection. No response at the drive thru speaker box

The following violations have been corrected:
14) Flouring area was cleaned and sanitized to reduce the grime build up that had been previously noted.
14) No chicken racks stored in the mop sink. Continue to utilize 3 compartment sink for all food contact equipment. Chicken racks shall be cleaned after usage.
23) No live or dead vermin observed at time re inspection. Facility shall continue to provide pest control.

The following violations need to be corrected:

The following violations have been corrected:
1) No thawing was observed at time of re inspection. Standing water was observed at the food prep sink but no chicken soaking in sink.
Thawing using standing water is not approved.
21) Hand wash sinks all contained warm water at 100 degrees F.
23) No signs of vermin observed. Continued to provide pest control. Increase frequency of visits if necessary.
33) Nonfood contact surfaces were observed to have been cleaned. Continue to maintain cleanliness of non food contact surfaces.
41) Step handles for hand wash sinks were observed to be functioning correctly.
41) Plumbing fixture underneath the hand wash sink near the walk in unit was repaired. No leak observed.
45) Cracked tile underneath the hand wash sink near the walk in unit.
The following violations still need to be corrected:
14) Pans located inside the walk in unit that did not have thawing meat were not removed from walk in unit and taken to 3 compartment sink. Pans shall be washed, rinsed, and sanitized every 24-48 hrs.

Observed warm water at followings sinks to be below 100 degrees F: -Hand wash sink near drive thru window -Hand wash sink near walk in refrigerator unit -Hand wash sink in men's restroom. ALl hand wash sinks shall provide warm water at 100 degrees F. Increase water temperatures at hand wash sink.

Pan used to thaw bagged raw chicken inside the walk in unit had not been cleaned and sanitized in two weeks. Remove pan and properly wash, rinse, and sanitize at 3 compartment sink. Pan shall be properly washed daily.

Observed poorly functioning step handles for all three hand wash stations in food prep area. Foot handles were getting stuck causing water to flow continuously. Repair or replace foot handles for all three kitchen hand wash sinks. Observed leaking piping at hand wash sink near the walk in unit. Repair piping to eliminate leak.

Observed raw chicken thawing in standing water at food prep sink Use only one of four approved thawing methods: 1) Inside refrigeration 2) As part of the cooking process 3) Under cool running water (70° F) for no longer than two hours 4) In a microwave, provided food is immediately prepared after thawing Raw chicken was discarded. See item #7.

Observed cracked tile underneath the hand wash sink near walk in unit causing water to accumulate under the foot handle of the sink. Repair or replace tile. Observed grime build up on the floor surfaces in the following areas: Flooring underneath the cooking equipment -Flooring under back storage racks -Flooring where grease trap in stored -Flooring underneath register counter Clean and sanitize all flooring throughout facility.

Observed multiple dead cockroaches in the back storage area near walk in units. Another dead cockroach was observed near entrance of the facility. Clean and sanitize flooring where cockroaches are observed. The last pest control receipt was dated for 5/4/2022. COntinue to provide pest control. Increase frequency if necessary.

Observed 30 lbs of raw chicken being thawed at the food prep sink in standing water. Temperature of chicken was measured at 60 degrees F. Employee stated that chicken had been thawing in standing water over night. Raw chicken was voluntarily discarded.

Employees and manager were asked what are the approved thawing methods and none were able to answer. Staff were educated on proper thawing methods at time of inspection.
