Note: Inspector checked on an old equipment only plan check (SR#48766) originally approved in 2018. Upon inspection, it does not appear that the plans proceeded. Plans related to updates to the 7-Eleven cook work station used for reheating foods hot held in warmers. Per employee, he is not aware of any changes planned to the reheating station. This old plan review will be closed at this time.

The vinyl sheet flooring throughout the back of house area used for utensil washing, food/utensil storage, the ice machine, etc. is deteriorated in several locations and is no longer smooth and easily cleanable. Per employee, the flooring is to be replaced soon when the store is upgraded.

***Replacement of flooring, and any other changes to equipment/layout needed for the food facility, requires plan review with this Division. Contact inspector for questions.

Thermometer was not provided at time of inspection. Provide probe thermometer for roller grill items immediately.

SECOND REPEAT VIOLATION
Observed debris and soda cans stored inside the 3 comp sink. Keep the 3 comp sink clean and organized at all times. The 3 comp sink shall be used for washing dishes only. This is a second offense of this violation, a third repeat of this violation may result in a compliance agreement.

Observed handsanitized instead of soap at the hand washing sink. Handwashing sinks must be properly supplied and accessible at all times.

Observed no toilet paper in both bathrooms. Keep facilities stocked at all times.

SECOND REPEAT VIOLATION
A) Observed vinyl sheet flooring by the warewashing area, in front of the ice machine and under the soda box rack no longer smooth, non-absorbent, durable and cleanable surfaces. Repair flooring by 05/26/2021. This is a second offense, a third repeat of this violation may result in a compliance agreement.

B) Observed build up and spills under the racks inside the walk in cooler. Keep floors clean.

Observed excessive mold build up in ice machine. Correct imediately.

Observation:
There was only 1 employee on site. Employee was not wearing face covering, but physical barrier was installed all around the front counter to protect the employee from possible exposure. Employee was wearing gloves while handing money and/ or food. Face covering and hand sanitizer is available on site for employee if needed. Hand sanitizer is available by the front counter for customer to use and markings were placed on the floor to keep customer 6ft apart. The self-service condiment station for relish, chili, etc is closed. Per manager, shared condiment such as creamer, sugar, etc and common touch area are sanitized every hour.

Reminder:
Failure to correct new or repeat violation may result in a yellow placard and/ chargeable re-inspection.
Please post signage by front entrance. Signage emailed to operator.
Highly recommend keeping the disposable lids for the soda fountain and coffee stirrer behind the counter to prevent customers from touching them. Customer may ask for them when they pay at the register.

A) Observed caulking behind the 3 comp sink in disrepair. Repair by 11/29/2020.

B) Observed large amount of cans, milk, etc stored inside the 3 comp sink. Miantain the 3 comp sink clean and organize. The 3 comp sink shall be use for washing dishes only.

Observed no wall mounted dispenser for the roll of paper towel by the front hand sink. Please provide a wall mounted dispenser by the front hand sink.

A) Observed the vinly flooring in the back area no longer smooth, durable, nonabosrbent and cleanable surfaces. Please repair by 11/29/2020. Please submit plans to PCEH if replacing with a different type of flooring.

B) Observed accumulation of debris and dirt under the racks in the walk in cooler/freezer and back area. Maintain floors clean.

B) Observed build up under the racks and equipment throughout the back and inside the walk in cooler, especially under the milk rack in the walk in cooler. Keep floors clean.

Observed no single use paper towel in the paper towel dispenser. A roll of paper towel was noted on the counter. Provide single use paper towels inside the dispenser.

A copy of the last routine inspection report was not available. Provide immediately.

Observed 2 spray bottle by the mop sink with clear and pink solution with no labels. Provide labels on all chemical bottles.
