Observed empty paper towel dispenser at the hand wash sink in the back kitchen. Observed empty soap dispenser at the hand wash sink at the front service counter (Auditorium 1-5 side) Observed empty paper towel dispenser at the hand wash sink at the front service counter (Auditorium 6-10 side) Observed jammed paper towel dispenser in the women's restroom. (closest to entry door) Soap and paper towels shall be supplied in dispensers at all times. (Note: inspection occurred during a school field trip and only 2 staff members were present at the time) Correct immediately.

Observed food debris and grime in-between and under equipment in the back kitchen. Please clean under and around equipment.

Observed hot water at 114 degrees F. Food facilities must provide hot water at a minimum temperature of 120 degrees F measured from the faucet (except handwash sinks which may be a minimum of 100 degrees F). Correct immediately. A re-inspection will occur on or about June 21, 2023. Failure to correct violation may result in a Yellow Placard and or a re-inspection fee.

Observed some biofilm slime/mold accumulation inside ice machine. Ice machine must be routinely cleaned and sanitized to prevent the growth of bacteria and microorganisms. Drain ice, clean and sanitize inside ice machine.

Facility does not have a current Certified Food Safety Manager. Food facilities that prepare potentially hazardous foods must have, at minimum, one Certified Food Safety Manager on staff. Obtain current Food Safety Manager Certification by July 19, 2023. Food handler cards were not available for review. All food handlers must have a valid food handlers card within 30 days from date of hire and records must be available for review. Correct by July 19, 2023.

Observed hot dogs in hot holding reading 135-125 °F. Maintain all hot held items at 135°F or above. Note: Per manager, facility uses time as a public health control. Log was incomplete (see #8)

Time and temperature log was not completed for PHF (hot dogs). First temperature check was to occur at 12:00, inspection occurred at 12:30 pm. When time is used as a public health control, daily records and documentation must be maintained. Correct immediately.
