This is a follow-up inspection concerning the air conditioning and the mouse in the facility. The air conditioning has been repaired and has been working for a week, the portable coolers have been removed. The pest control company has removed the snap traps from the ledge above the storage racks. Employee stated that the bread and other items have been left out and they have not seen any evidence of the mouse and the pest control company is doing routine maintenance and has not seen any other rodents in traps. The walk-in cooler has been repaired and no longer has an issue with ice build-up and water leaking from pipes. Unit is clean and free of debris.

The following needs to be addressed:
Due to the amount of time that has gone by without the air conditioning working properly the facility will be issued a YELLOW PLACARD. There are portable cooling units in the facility, these are not allowed in a food service area, please remove them. Another issue that came up is the rodent problem, a cardboard box contained a hole from an apparent rodent and there are snap traps placed in the facility above the dry storage racks. Please remove the snap traps and place bait traps instead, snap traps are not allowed. The walk-in cooler has an issue with ice build-up, please have unit serviced to make sure it stays at a constant temperature at all times.

I have tried to contact the owner numerous times and have spoken with a representative at the main office, but have yet to receive a call back or email from owner. I did speak with an HR representative on 9/19/19 and she stated that it was the first time she had heard about the issue. The air conditioning went out around 8/20/19, it has been a month and with the oven operating around 483*F it makes it really uncomfortable for the employees.

The YELLOW PLACARD will remain until the air conditioning gets repaired properly and the issue with the walk-in and rodents get addressed. The pest control business needs to replace the snap traps and the walk-in needs to be professionally to avoid ice build-up of cooling unit and avoid having to defrost and shut down unit.

Facility has had an issue with the air conditioning unit not working and apparently it was fixed, but recently stopped working. Please properly repair air conditioning, pizza oven operates at 483*F and makes the kitchen very hot when combined with the warm temperatures outside.

I observed some rags on floor of walk-in cooler, cooling unit drips and has ice build-up causing unit to not cool properly. Unit then has to be defrosted and temperatures in unit increase above 41*F. Unit must be repaired and keep a constant temperature of 41*F or below.


Observed several snap traps and a cardboard box of bread with a hole in it in the back area. Facility has an issue with rodents. Pest control business services facility and will be stopping by later today. Please have snap traps removed and place bait traps instead. Relocate any food items to avoid rodents from getting into them.
