No test strip available for high temp warewasher. Provide high temperature test strips to verify warewasher is reaching proper sanitizing temperature.

Observed significantly worn and a cracked whisk handles and rubber spatula. Replace equipment and utensils as they become worn and no longer cleanable.

Observed: A large metal salad bowl was stored directly on top of the lettuce, inside of the salad prep unit, with the bottom of the bowl touching the lettuce. A cup was being used as a scoop inside of the Quesadilla Salsa, located in the kitchen walk in cooler.

Corrective Action: Discontinue the practice of storing salad bowls on top of lettuce inside the salad prep unit. Discontinue the use of using cups as dispensers for any food.

Observed: The ice machine in the main kitchen has mold. The meat slicer is dirty with old meat debris. The filters above the main cook line are dirty.

Corrective Action: Clean and sanitizd the ice machine to remove mold. Clean meat slicer to remove dried on meat debris. Clean the filters above the main cook line to remove grease.

Observed: All grab and go items meant for immediate consumption were not properly labeled. Only the common name were listed on the label.

Corrective Action: Grab and go items are required to have labels with the following:
1.The common name of the food (for example, “Breakfast Burrito”)
2.List of ingredients - If made from two or more ingredients, the list of ingredients shall be in descending order of predominance by weight, and include a declaration of artificial color or flavor and chemical preservatives contained in the food.
3.An accurate declaration of the quantity of contents (weight of item – this may not apply to all items)
4.Name and address of the facility where the item was made
5.An allergen statement. If a food contains milk, egg, peanut, tree nut, wheat, soy, fish, or shellfish then the label must say “Contains ______ (fill in the blank with allergen)”
This a repeat violation. To avoid a conditional placard and possible re-inspection fees this must be corrected immediately.
