09/09/2021: Routine Inspection

1. Clean fan guards in walk in refrigerator. Repair leak from fan.
2. Discontinue practice of lining shelves with cardboard boxes. Use a material that is smooth, cleanable and nonabsorbent.
3. Repair lid on prep refrigerator when cover is coming loose.
4. Clean exteriors of cooking equipment.

Chain Co2 canisters to prevent them from tipping over.

Food containers observed in kitchen stored directly on ground. Keep all foods stored at least 6" off of ground.

Clean hood filters.

Provide 1" air gap at all floor sink drains. Multiple pipes were observed to have fallen into sink basin.

Provide consumer advisory on menu for items that can be ordered cooked to less than required temperatures.

Food safety manager certification is expired. Provide new certificate within 60 days for one employee. Multiple food handler cards were missing/expired. Maintain copies of food handler cards for all employees onsite.

1. Floors are walls throughout facility are in need of thorough cleaning. Especially under and behind equipment and bar.
2. Repair broken and loose base coving tiles under hood.

Inspection Memo:

Observed hot water @ 121F.
Observed cold holding temperatures @ or < 41F.
Hand-washing facilities fully stocked.
Observed proper sanitizer levels.

• Facility had implemented a Workplace Plan but did not have it in writing. Workplace Plan was written during inspection.
• Employees will be trained on written Workplace Plan and training will be documented.
• Observed employees wearing face coverings.
• Observed physical distancing and COVID-19 signs posted.
• Per Plan, high touch areas are sanitized [hourly].
• Facility has reduced the seating capacity of the onsite dining areas to provide a minimum of 6 foot physical distancing.
• Observed hand sanitizer provided at entrance to facility.
• [Disposable/Digital menus] are being used.

Asked chef what temperature burger patties were going to be cooked at.
Cook responded that the pattie would be cooked to 145F.
Advised employee that burger patties need to be cooked to 155F unless otherwise requested and menus have a consumer advisory.
Train employees on proper food temperatures.

Secure CO2 tanks to prevent tipping.

Provide consumer advisory to menu for items that can be ordered cooked to less than required temperature.

Proper hand washing not observed due to violation #6 below.

Clean hood and filters. Arrange filters on hood so there are no gaps.

1. Clean refrigerators, inside and backs of handles.
2. Clean equipment under hood.
3. Clean fan in walk in refrigerator.

1. Floors and walls throughout kitchen and bar are in need of a thorough cleaning.
2. Fix base coving tiles under hood that have separated from wall.

Hand wash sink basin observed blocked with food container, and paper towel dispenser was empty. Basin was emptied and paper towels refilled during inspection. Keep sink accessible and stocked at all times to allow for proper hand washing.

Clean floor sinks in bar and kitchen.
