1.) Observed warewash machine at 200ppm chlorine. Have unit serviced to produce 50ppm chlorine during rinse cycle.

In the meantime, sanitize all dishes and utensils in the 3-compartment sink.

2.) Sanitizer dispenser is making 400 ppm. Have unit serviced. Make up buckets by hand in the meantime.

Note: Inspector worked with manager to create a ratio of sanitizer and water that produces 200 ppm quat. Please fill sanitation buckets a quarter of the way and fill the rest with water; recorded at 200ppm quat. CORRECTED ON SITE.

1.) Observed can opener by meat slicer with significant food debris. Please maintain can opener to prevent food debris accumulation.

2.) Observed significant rust accumulation on wire racks in dry storage area. Please maintain wire racks to prevent rust accumulation.

3.) Observed dust accumulation on ceiling of walk-in cooler. Please maintain walk-in cooler to prevent dust accumulation.

REPEAT VIOLATION: SECOND NOTICE

1.) Observed two pressurized cylinders by employee entrance and one pressurized cylinder by cashier area (CO2) not secured to a rigid structure. Please secure all pressurized cylinders to a rigid structure ASAP.

2.) Observed outlet pulled out from wall in dry storage area. Please repair/replace outlet ASAP

REPEAT VIOLATION

1.) Observed toilet tissue sitting on hand rails for both restrooms. Please train employees to properly restock toilet tissue ASAP.

Note: Per manager, toilet tissue cannot fit inside current toilet tissue dispenser. Inspector discussed with manager on different options to correct violation.
