Observed mold accumulation in the bottom of the reach in freeze in the back storage area. Clean and sanitize.

Observed debris accumulation in the bottom of the reach in refrigerator in the back storage area. Clean.

Observed multiple areas in the back storage area with several rodent dropping. Thoroughly clean and sanitize contaminated area using proper PPE. Contact certified pest control operator to mitigate issue and continue to monitor for activity.

- Facility has a written workplace specific COVID19 prevent plan, however it was not available at the time of inspection. Please print workplan and maintain on site at the facility. Email copy of workplan to inspector.
- Observed all employees wearing face coverings
- Employees are trained not to come to work if not feeling well
- Verbal wellness checks perform prior to starting shift
- Hand sanitizer provided for employees and customers
- Restrooms, tables, high traffic surfaces sanitized every 2 hours
- Tables spaced to facilitate distancing
- Visual cue markers provided on the floor
- COVID19 signage posted at entrance

Observed shell eggs stored above lettuce in the reach in refrigerator in the wood room. Eggs must be stored below ready to eat food to prevent cross-contamination. Move eggs to the bottom shelf. Corrected on site.

Observed bags of onions stored on a shelf in the wood room. This area is not vermin proof. Move onions inside main facility or provide a vermin proof storage container. Correct ASAP.

Observed white gallon container labeled "Final Rinse," per employee product is chlorine sanitizer. Working containers used to stored chemicals must be clearly labeled with the common name of the product. Add "Chlorine Sanitizer" to container label.

Observed crumbs and debris accumulation in the bottom of all refrigeration unit. Clean and maintain. Comply by 2.14.2020

Observed unused/inoperable ice machine. Equipment must be fully operational or removed from the facility. Correct by 2.27.2020

Observed chlorine sanitizer bucket at <30ppm. Sanitizer concentration must be prepared to 100ppm chlorine and be changed out
every 4 hours. Use chemical test strips to verify sanitizer is prepared to the proper concentration. Corrected on site.

Observed equipment shelves positioned less than 6 inches above the floor with debris accumulation underneath. Food and equipment must be storage at least 6 inches above the floor to facilitate cleaning and prevent cross-contamination. Adjust shelves to at least 6 inches high.

Observed chemicals and cleaning agents stored directly on the floor under the 3-comp sink. Chemicals, equipment, food, and cleaning agents must be stored at least 6 inches above the floor. Correct ASAP.

REPEAT VIOLATION - Facility does not have a current certified food safety manager. All facilities that serve potentially hazardous foods must have, at minimum, one employee with a current food safety manager certification. Obtain current certification by 5.1.2020. (Note: the original certificate must be kept on site and made available upon request)
