Note: Nozzles were cleaned while inspector was on site. CORRECTED ON SITE.

1. Observed raw chicken stored above ready to eat items. Item was moved to lowest rack. CORRECTED ON SITE.

Observed full unchained pressurized cylinders. Attach cylinders to rigid structure using chain.

Walls surrounding pizza mixer have serious damage and are not easily cleanable.

Note: Per Mundo, he has tried to clean walls but debris does not come off. Facility should consider covering wall with FRP material so that it is easily cleanable.

REPEAT VIOLATION
1. Observed significant grime build up on floors underneath 3 compartment sink and underneath refrigeration units, dry racks, and food prep surfaces. Facility must deep clean floors and walls.

Observed dirty racks in dry holding area. Clean.

Observed chicken at 45*F and sausage at 45*F in top holding of refrigeration unit. Tomato inside reach-in compartment of same unit was at 48*F. Outside temperature reading of refrigeration unit was 57*F at time of inspection. Potentially hazardous food (PHF) must be kept at 41*F or below. Have unit serviced to ensure PHF is at 41*F.

Note: Manager Mundo said unit was serviced a few weeks ago. Must moniter PHF if unit to ensure it is at 41*F.

1. Observed significant food debris accumulation on walls and floors around dough mixer. Clean.

Observed no paper towels in restroom. Restock paper towels.

Observed significant grime build up on floors underneath 3 compartment sink and reach-in refrigeration units. Clean.

At time of inspection a small leak in ceiling was observed leaking onto the floor at the entrance of the food prep area. During inspection water was being collected and removed from facility. Spoke with manager mundo about leak and he said they're waiting on building manager to repair it. No food is threated to be contaminated by leak.

1. Observed significant grease accumulation in trough underneath 3 compartment sink. Clean

2. Observed dirty racks and accumulation of food debris in True brand refrigeration unit. Clean

Observed 3 compartment sink draining directly into trough without a 1 inch air gap to prevent backflow. Lower trough to provide 1 inch air gap.

Note: Spoke with manager Mundo about lowering trough and said h will have an employee adjust it.

Observed significant food debris accumulation on floors/walls around the dough mixer. Clean

Observed temperature at 3 compartment sink at 116*F. Adjust heater to provide water at 120*F.

- Observed all employees with pony tails/ hair nets or hats.
- Observed cheese and pepper shakers for each table clean and maintained.
- Observed salad caprese plates clean and stored in a manner to prevent dough/sauce from getting on them.

2.) Observed Ice dispenser for soda machine with mold accumulation. Please maintain ice dispenser to prevent mold accumulation.

Note: Discussed with owner about maintaining ice dispenser.

3.) Observed flour accumulation on cold holding units by food prep line. Please maintain units to prevent flour accumulation.
