Reinspection of major violations noted at previous inspection.
-Pest control has been performed. Copy of invoice provided.
-Cleaning has been conducted. Continue cleaning hard to reach/see areas.

* Refrigerated drawers under grill observed at 48*. Potentially hazardous foods were discarded that had been in refrigerator since yesterday. Other items were moved into another refrigerator. Do not use refrigerator until it can maintain a temperature <41*.

Complaint inspection conducted at time of routine inspection.
Complaint received alleged unsanitary conditions and dead cockroach observed.
Correct cleaning and pest control violations noted within one week. Reinspection to be conducted, if not corrected, yellow placard and/or reinspection fee will be required.

1. Clean baffle on ice machine.
2. Replace broken gaskets on refrigerators.

Clean exteriors of cooking equipment, handles on refrigerators and faucets, shelving in refrigeration (including walk in) and exteriors of food storage containers.

Chemicals observed stored on shelves in dry storage area. Keep chemicals stored properly separated from foods.

1. Undercounter refrigerator at sushi bar observed at 45*. Thermostat was lowered during inspection. Monitor temperature, if temperature cannot be maintained <41*, do not use for storage of potentially hazardous foods.
2. Two door refrigerator in kitchen observed at 48*. Potentially hazardous foods were removed from refrigerator and thermostat lowered. If temperature cannot be maintained <41*, do not use to store potentially hazardous foods.

Floors, walls and ceilings throughout kitchen are in need of thorough cleaning. Especially under and behind equipment.

Dead cockroaches observed on sticky trap in dry storage area. Have licensed pest control perform service and provide copy of report to Environmental Health office. Cockroaches were not observed in any other areas of facility.

Inspection Memo:

Facility is not offering onsite dining at this time. If onsite dining areas are opened, a Workplace Specific Plan shall be prepared and implemented. A Workplace Specific Plan template was provided to operator at the time of the inspection.

*Multiple areas of kitchen are in need of thorough cleaning- under and behind equipment, food storage containers etc. Thoroughly clean these areas.

1. Replace broken gaskets on refrigeration.
2. Replace duct taped lid on chest freezer.

Clean flooring under cooking equipment.

Provide accurate thermometers in refrigerators.

Clean duct in hood over dish washer.

1. Paper towel dispenser at hand wash sink in kitchen empty. Employees were using paper towel hanging from edge of rack. Refill paper towel dispenser. Keep filled at all times to allow for proper hand washing.
2. Soap for hand wash sink was in squeeze bottle. Use pump type soap dispensers.

1. Clean/replace racks in walk in refrigerator.
2. Clean equipment under prep table.

1. Sanitizer buckets for sushi bar observed <100 ppm quat ammonium. Sanitizer buckets were remade during inspection. Change sanitizer buckets often to ensure they are at the proper concentration. Provide test strips for checking sanitizer level.
2. Keep enough sanitizer solution in bucket to completely submerge towels.
