Observed bags of ice to be un-labled at facility. Ice is being bagged at facility, bags of ice must have labels of facility address and name for sale of ice.
Informed the facility has order new bags with store labels manufactured on them.

Observed Ice freezer to be malfunctioning, water is leaking from machine. Staff are aware and a service order has been placed.

Observed leak in piping above three comp sink near ceiling. A plastic bucket is on floor to catch leak water. Owner stated a work order has been issued for repair.
Observed three comp sink drain line below the level of the floor sink, please have drain line with an air gap to floor sink of at least one inch.
