Hot water shall be supplied at a 3 comp sink at a minimum of 120F.
Correct immediately.

Observed no paper towel at either hand washing station at time of inspection.
All hand washing facilities shall be supplied with wall mounted paper towel dispensers.
Paper towels were restocked at hand washing facilities nearest cook line.

Observed no soap at dispenser near 3 comp sink.
All hand washing facilities shall be fully stocked with wall mounted soap dispensers.
Correct immediately.

Observed sanitizer bucket on cook line with a concentration of less than 50ppm Chlorine.
Sanitizer buckets shall be prepared with a concentration of 100ppm.
Corrected on site.

HAND WASHING STATIONS - Fully Stocked however paper towels should be in the wall dispenser
HOT WATER SUPPLY - 120F+ three compartment sink, 100-108F hand washing sinks
SANITIZING - 400 ppm quaternary ammonia in spray bottle, 100 ppm chlorine in 3 compartment sink sanitizer basin
TEMPERATURE CONTROL - All potentially hazardous foods observed to be at or below 41F or at or above 135F
FOOD SAFETY TRAINING - Managers certificate and food handlers cards on site and current

• Employee on site could not find the work site plan but believed the manager had completed it. A new plan was filled out on site. Staff are following the COVID-19 state guidelines.
• Observed employees wearing face coverings.
• Observed physical distancing and COVID-19 signs posted.
• Observed barriers installed at point of sale.
• Per Plan, high touch areas are sanitized after each party.
• Facility is following the current "no dine-in" order. There is some seating outside which is in accordance with the physical distancing and air flow requirements.
• Observed hand sanitizer provided at entrance to facility.
• Employees have been trained on the contents of Plan. Training shall be documented on the plan.
• Regular menu's are being used but sanitized after each party.

Observed nonoperational paper towel dispenser in the cook line area. Repair paper towel dispenser ASAP. Second violation.

Observed employees washing hands in food prep sink and hand washing sink is blocked by garbage bin. Employees must wash hands only in designated hand washing area. Avoid such practice. Do not block hand washing sink with cart or garbage bin.

Provide food handler card for an employee ASAP. Comply by 12/31/2019.

Please organize and store chemical or foods roperly.

Observed chlorine sanitizer provided at 10 ppm for surface cleaning. Provide 100 ppm bleach/chlorine for surface cleaning at all times.
