Clean floors under and behind equipment.

No sanitizer was observed prepared along cookline. Sanitizer was made during inspection. Keep sanitizer prepared at proper concentrations.

1. Clean backs of handles on refrigerators.
2. Clean shelving under broiler.
3. Clean exteriors of cooking equipment.
4. Clean filters on hood.

One door true refrigerator in back observed at 46*. The only potentially hazardous items in this refrigerator were sliced tomatoes. Tomatoes were discarded during inspection. Do not use this refrigerator to store any potentially hazardous foods until temperature can be maintained <41*.

Dead cockroaches observed in sticky trap under bar. Facility has pest control service. Contact pest control for service and provide copy of report within one week.

Sliced beef observed in refrigerator stored above tortillas. Keep raw meats stored below or properly separated from other foods.

Provide food safety manager certification for one employee within 60 days.
