Facility has not prepared a Workplace Specific Plan. Preparation and implementation of Workplace Specific Plan is required for onsite dining areas to be open. Workplace Specific Plan template to be emailed to facility owner.
• Observed employees wearing face coverings.
• Observed physical distancing and COVID-19 signs posted.
• Observed barriers installed at point of sale..
• Facility has reduced the seating capacity of the onsite dining areas to provide a minimum of 6 foot physical distancing.
• Observed hand sanitizer provided at entrance to facility.
• Menus are being disinfected between uses.

Clean undersides of shelving in walk in refrigerator.

Store ice scoop so handle is not touching ice.

Repair leak on three compartment sink sprayer, and on faucet of mop sink.

Clean blade on can opener. Dried on food debris was observed.

Quat sanitizer observed >400 ppm. Use test strips to verify sanitizer level of 200 ppm when preparing sanitizer.

Salad bar temperature observed at 44* (salad dressings, shredded cheese). Lower temperature to 41* or below. If temperature cannot be maintained <41*, do not use to store any potentially hazardous foods.

1. Clean backs of handles on refrigerators.
2. Clean shelving in walk in refrigerator.
3. Clean meat slicer of food debris.

Store scoops so that handle is not touching food. Scoop in flour observed with handle buried in flour.

1. Clean flooring under and behind equipment in kitchen due to buildup of food debris.
2. Clean spilled soda syrup in dry storage room.

Clean floor sink under soda machine.

Paper towel dispenser at hand wash sink next to three compartment sink observed empty. Refill dispenser, keep paper towel dispensers filled at all times to allow for proper hand washing.
