12/17/2024: Routine Inspection

MAJOR VIOLATION - REPEAT VIOLATION - Observed rodent droppings on floor along back wall of storage area, and on/behind CO2 cylinder by walk-in cooler. Inspector was provided with copies of pest control reports, last service 12/12/24. Continue with regular pest control service and follow all recommendations. Seal all holes or points of entry. All rodent droppings were cleaned up at time of inspection. CORRECTED ON SITE.

Observed hole in ceiling in middle of warewash area. Per owner, roof has a small leak in this area. Repair ASAP. Per owner, in contact with landlord regarding repair.

NOTE: Location of leak does not appear to currently be affecting food items/utensils. If leak worsens, or affects the food prep area or warewash area directly, facility may be subject to closure.

1. REPEAT VIOLATION - 2nd Citation - Facility does not have a current Certified Food Safety Manager. Obtain current Food Safety Manager Certification ASAP.

2. Food Handlers cards are not available to review for all employees. Have all employees obtain a valid food handlers card ASAP, maintain copies on site.

Observed broken thermometers in one prep top refrigerator and walk-in cooler, and no thermometer in other prep top. Ensure all units have functioning thermometers.

1. MAJOR VIOLATION - Observed "cowboy chili" being held in a turned off slow cooker. Chili probed at 90*F. Food must be held at or below 41*F, or at/above 135*F. Per owner, exact time slow cooker had been turned off was unknown. Chili voluntarily discarded. Inspector provided education on holding temperatures. CORRECTED ON SITE.

2. Observed one prep top refrigerator holding food (tomatoes, chicken) at 44*F, and the other prep top refrigerator holding food (chicken, dressings) at 44*F. Potentially hazardous food (PHF) must be held at or below 41*F. Both refrigerators were turned down at time of inspection. If unit does not hold food at or below 41*F, move food to alternate refrigerator and have units repaired. CORRECTED ON SITE.

Observed small smoker unit by front door. Barbecues must be separated from public access to prevent contamination and for safety. Provide barrier/signage to separate smoker from public area.

Routine Inspection.
Due to evidence of rodent activity, a Yellow Placard has been posted. Facility must be cleaned & sanitized (no evidence of rodent droppings) before opening tomorrow (8/6/24). Inspector will perform reinspection prior to facility opening. Call pest control to service facility, send inspector pest report by 8/9/24.

Failure to comply in the above may result in closure of the facility and reinspection fees.

MAJOR VIOLATION - Observed both hand wash sinks to not be stocked with paper towels or soap. Handwash sinks must be supplied and accessible at all times. CORRECTED ON SITE.

REPEAT VIOLATION - 2nd Citation - Facility does not have a current Certified Food Safety Manager. Obtain current Food Safety Manager Certification ASAP.

MAJOR VIOLATION - Observed sanitizer buckets at 0ppm. Maintain sanitizer at 200ppm quaternary ammonium or 100ppm bleach. Change out sanitizer solution as often as needed to maintain correct concentration. CORRECTED ON SITE.

MAJOR VIOLATION - Observed rodent droppings under 3-compartment sink, on non-food items on/behind bottom shelving, and behind CO2 tank. Facility does have pest control, last report dated 7/20/24. Facility must be maintained free of pests and vermin.
Inspector will return on 8/6/24 prior to facility opening to ensure all rodent droppings have been cleaned up. Call pest control company, send inspector report by 8/9/24. YELLOW PLACARD POSTED.

Provide thermometers in prep top refrigerators.

Inspection Memo:


On-site for reinspection of violation regarding walk-in refrigerator, noted on routine inspection report dated 3/5/24 and verification inspection report dated 3/8/24.

Observations:
Walk-in refrigerator was repaired two hours previously, holding an ambient temperature of 44*F. Per repair guide, refrigerator may take 3-5 hours to reach correct operating temperature.
A photo of the thermometer was sent to inspector 2 hours later, verifying ambient temperature at 37*F.

In compliance, violation corrected. Facility permitted to use walk-in refrigerator to hold potentially hazardous food.

Note: Per verification inspection report dated 3/8/24, trash behind facility to be cleaned up by 3/18/24. Failure to comply will result in a Yellow Placard and reinspection fee of $448. Send inspector photo of dumpster area.

REPEAT VIOLATION - Facility does not have a current Certified Food Safety Manager. During previous inspection, this was in progress (proof of enrollment sent to inspector), and now business is in the process of a change of ownership. Obtain current Food Safety Manager Certification ASAP. Inspector will verify during next routine inspection.

Observed walk-in refrigerator holding an ambient temperature of 63*F. Per owner, unit has been broken for 3 days and is being fixed on Friday (waiting for parts). Observed lunch meat stored in large ice chests filled with ice in walk in. Meats were below 41*F. Inspector had owner transferred meats to other functionable prep refrigerators in main kitchen area. All other potentially hazardous food (cheese) was already in alternate refrigerators. Facility has enough refrigeration to remain open during this time. MITIGATED ON SITE.

Reinspection required. Inspector will return on/about 3/8/24. Do NOT use walk-in refrigerator to hold potentially hazardous food until repaired and reinspected.

Observed spillage in walk-in cooler under beer kegs. Clean.

Observed several flies in facility in warewash area. Take actions to prevent/eliminate flies in facility.

Observed used wiping clothes lying on prep area. Wiping clothes must be kept submerged in a bucket of sanitizer solution. CORRECTED ON SITE.

REPEAT VIOLATION - No food safety manager certification available at the time of inspection. Facility must have, at
minimum, one employee with a valid food safety manager certification. Certificate must be kept on site and made available upon
request.

*Note: Inspector provided with proof of enrollment during previous follow up inspection on 5/2/23. Course is still in progress. Facility must have valid certification by next routine inspection. Failure to comply may result in Yellow Placard and re-inspection fee.

REPEAT VIOLATION -Observed empty paper towel dispenser at the handwashing sink closest to the office. Paper towels must be supplied in a dispenser at all times. Provide paper towels in a dispenser ASAP.

Note: alternative hand washing sink supplied and accessible close to prep area.

Observed lights to not be functional in back dry storage area. Adequate lighting must be provided in facility to facilitate cleaning and inspection. Correct ASAP.

Due to outstanding repeat violations, a MAJOR VIOLATION has been issued for failure to provide a Food Safety Manager certificate. See Violation #1

This office will return on or around 5/3/23 to verify that the MAJOR VIOLATION has been corrected. Failure to comply may result in a yellow placard and re-inspection fee of $417. Correction may be emailed to inspector before this date.

1. Observed sanitizer bucket at 100 ppm quaternary ammonium sanitizer. Maintain at 200ppm. Inspector educated employees on proper concentration of sanitizer solution and the use of test strips. CORRECTED ON SITE.

2. Observed quaternary sanitizer in 3-compartment sink for manual warewashing at 100ppm. Maintain at 200ppm. CORRECTED ON SITE.

1. Observed containers of uncovered bacon above the main prep tables and on top of the microwave. Keep food covered to prevent contamination. CORRECTED ON SITE.

2. Observed cardboard boxes of whole avocados and tomatoes stored on the floor in the walk in cooler. Store all food at least 6" off of the ground. Boxes moved to a shelf during inspection. CORRECTED ON SITE.

Observed drain line from beer tap draining into a 5 gallon bucket in the walk in cooler. All liquid waste must discharge to a floor sink or other approved plumbing system.

REPEAT VIOLATION -4th CITATION- No food safety manager certification available at the time of inspection. Facility must have, at minimum, one employee with a valid food safety manager certification. Certificate must be kept on site and made available upon request. Email inspector copy of valid certificate or proof of enrollment in approved course by 5.3.2023. Failure to comply will result in a yellow placard and reinspection fee.

Observed fly strip above hand wash sink in between main prep area and warewash area. Fly strips are not permitted for use in food facilities. Insect control devices must retain the insect within the device. Remove fly strip ASAP and install approved insect control device if necessary.

08/24/2022: Routine Inspection

1. Observed grime and dust accumulation on shelving in walk in cooler. Clean.

2. Observed mold and dust accumulation on ceiling in walk in cooler. Clean.

Observed trash and debris accumulation outside on the ground behind the facility. Premise must be kept clean and free of debris. Per employee, trash is from neighboring facility that went out of business, as the back area of the facility is shared. Coordinate with appropriate parties to thoroughly clean area. Correct ASAP.

MAJOR VIOLATION
Observed 3-compartment sink with 0ppm quaternary ammonia sanitizer and active warewashing taking place. Per employee, company serviced automatic sanitizer dispenser the previous evening. No tub of sanitizer was visible under sink. Call company to replace sanitizer and ensure automatic dispenser is working properly. Ensure quaternary ammonia test strips are on site.
Employee found a small amount of quaternary ammonia sanitizer and put into sanitizer compartment of 3- compartment sink. Concentration measured at 200ppm.
Employees will use chlorine sanitizer at 100ppm for ware washing and sanitizer buckets until automatic dispenser is working properly. Chlorine and chlorine test strips are on site. CORRECTED ON SITE.
