1.) Observed no sanitizer bucket in facility. Provide a sanitizer bucket at 100ppm chlorine or 200ppm quaternary ammonia. Note: employee set up sanitizer bucket while inspector was on site; recorded at 200 ppm quat. CORRECTED ON SITE.

-Face mask and 6-feet physical distancing signs observed.

05/04/2021: Routine Inspection

 Minor - 3

-Observed Quat sanitizer in spray bottle at 500 ppm. Maintain quat sanitizer at 200 ppm Manager diluted with water between 200 and 400 ppm. Inspector showed manager how to prepare 5-gallon bucket with quat sanitizer. Corrected on site.

-Observed a bag of onions on the floor of walk-in cooler. Please store all foods at least 6" off the floor.

-Observed significant grease build up on hood grease filters. Please maintenance hood to prevent grease build up. Per manager it has been approximately 9-months since hood was last maintenance. Manager stated hood will be maintenance sometime this week.
-Observed tin foil on pizza oven with food debris accumulating. Per manager tin foil is used as a form of keeping the pizza oven clean, he stated that the tin foil is replaced once a week. Please replace tin foil more frequently to prevent build up food debris on pizza oven.

Observed Quat sanitizer in spray bottle at 500 ppm. Diluted with water between 200 and 400 ppm. Maintain quat sanitizer at 200 ppm.

Observed back hand sink without paper towels and blocked by the dough mixer. This is a first repeat violation. Dough mixer was moved to the side and paper towels were restocked. Maintain a stocked and accessible hand sink at all times.

Observed heavy grease and dust on exhaust hood and filter panels. This is a first repeated violation.
Observed air venting going into the building with a bag and tubing to direct the air to the oven. Air venting must be unobtructed.

Observed caulking on restroom hand sink in disrepair. Repair. Correct by 3/17/20.

Observed back hand sink blocked with dough mixer. Maintain hand sink clear and accessible at all time.
Corrected on site.

Observed heavy grease build up on exhaust hood and filter panels. Clean exhaust hood and filter panels. Correct by 3/17/20.

A) Observed multiple bins on the prep table containing white powders with no labels. Provide labels. Correct immediately.
B) Observed boxes of food stored on the floor in the walk in cooler. Store food at least 6 inches off the floor. Correct immediately.

Observed one employee food handler card expired 2018. Have employee obtain a valid food handler card immediately.

Observed dust build up on fan covers inside the Pepsi refrigerator. Clean fan covers. Correct by 3/17/20.
