REPEAT VIOLATION No sanitizer was prepared when inspector arrived at facility. Note: Manager prepared sanitizer bucket while inspector was on site. CORRECTED ON SITE.

Observed boxes of italian sausage being stored on ground of walk-in refrigerator. Food must be stored 6 inches off the ground. Place boxes on wire rack. Note: Per manager delivery had occurred today and deliver driver from Sysco placed items on floor and slid them underneath wire racks.

Observed uncovered mushrooms in walk-in refrigeration unit. Cover food to prevent from overhead contamination.

Current Food Safety Manager's Certificate is expired. Acquire new certification ASAP.

Observed dirt and debris caked onto wall behind 3 compartment sink, handwash sink, and dough making area. Clean. Observed large accumulation of flour on floor underneath dough preparation area. Clean.

Observed overstocked mozzarella in top holding refrigeration unit at 46*F. Do not fill items above fill line in containers. Note: Manager stated that he overfilled to prepare for the lunch rush.

REPEAT VIOLATION 1. Observed significant food debris build up on wire racks in walk-in cooler. Clean.

Observed 2 metal spoons stored on top of electrical panel. Note: Manager was unsure why they were stored there, proceeded to discard metal spoons. Inspector explained to manager to keep sanitized utensils on storage racks that facility has. CORRECTED ON SITE.

Observed no sanitizer bucket in facility. Provide sanitizer bucket at 200ppm quaternary ammonia. Note: Inspector arrived at 12:30pm for inspection, as inspector arrived manager made sanitation bucket and put it next to the reach-in refrigeration unit. Facility had been open since 11am. Inspector spoke with manager about making sanitation bucket as part of opening routine in order to have it ready before any food prep occurs. CORRECTED ON SITE.
