REPEAT VIOLATION: Observed ice build-up in walk in freezer, clear away ice to prevent possible accidents.

Observed Nitrogen tank without chain. CO2 Tank has chain. All pressurized containers must be secure to prevent possible injury.

Observed dust build up on restrooms vents. Clean affected area.

Observed hand sink by soda baffle to not be able to turn off the hot water properly. Repair hot water nob.

Observed fruit flies in the bar area. Clean area and contact pest control as needed.

Observed open container of fish fillets in cold holding lower storage. Keep all stored potentially hazardous foods covered to prevent possible contamination. CORRECTED ON SITE.

1. Observed ware-washing machine to be at 0 ppm chlorine. Bucket of sanitizer was empty.
Container was changed out and machine primed, observed sanitizer to now be at 100 ppm chlorine. CORRECTED ON SITE.

2. Observed sanitizer bucket at ice cream station to be at 0 ppm quant. Maintain 200-300 ppm quant.
Bucket was changed out, observed to be at 200 ppm quant. CORRECTED ON SITE.

Observed sanitizer in the ware-washing machine of the bar area to be at 0 ppm, container was noticeably low. Changed out container and retested at 50 ppm chlorine. CORRECTED ON SITE.

Observed ice build-up in walk in freezer, clear away ice to prevent possible accidents.

Observed grout deterioration in back ware-washing area. Repair grout.

Observed snap-mouse traps in back area. Contact pest control provider and have proper approved traps placed as needed.

Observed a few fruit flies in the back of the facility near mop sink.

Take the following precautions to help eliminate flies: Clean and sanitize damp, dark areas in the kitchen (floor sinks, underneath the dishwasher/3-comp sink).

Observed small gas canister without chain, all high pressurized canisters shall be chained down to prevent possible accidents.

Observed brooms resting in mop sink, have all brooms stored out of wet environments.

Observed slight mold build up on the wall of the first ice machine. Drain, clean, and sanitize.

Observed drainage pipes in the bar to be below the rim of the floor sink. Provide an air gap that is twice the diameter of the tubing (minimum 1").

Observed plastic bin of cole slaw nesting over tomatoes in top cold holding unit across from cook line, please discontinue practice of placing containers over food items. Corrected on site.


Observed freezer door not closing properly, please have door adjusted to be able to close correctly.


Observed employee drink on rack near dried goods area without a straw and lid. Please have an area for employee drinks and make sure cups have a straw and lid. Corrected on site.


Observed plastic drain line going into hand sink to the right of cook line. Employee stated that its a drain line that was connected to the AC unit above. Please remove pipe and discontinue practice of placing drain lines into sinks. Employee stated it will be removed.
