Spray sink at the kitchen ware wash machine is draining into a floor sink without a 1 inch air gap.
Create a 1 inch air gap at all indirect waste connections.

Observed employee drop lid to a food container on floor surfaces and placed it back over container without proper wash, rinse, and sanitizing.
ANy food contact utensils and equipment that has been contaminated shall be washed, rinsed, and sanitized before being used again.

Large wall opening was observed underneath the kitchen ware wash machine. Patch hole for smooth and easily cleanable surfaces and for vermin proofing.

Facility did not have appropriate testing methods for the high temperature ware wash machine. Provide thermo labels are high temperature thermometer to ensure final sanitizing phase is reaching a minimum of 160 degrees F.

Fly trap was placed over a food contact surfaces. Place fly trap on wall surface away from any food items or contact surfaces.

Soap and paper towels at the front hand wash sink in the meat prep area were not stocked inside dispensers.
Provide soap and paper towel dispensers and stock accordingly.
